Licensing Program Analysts (LPA) Denise Gibbs conducted an unannounced annual required inspection at the above facility on 1/20/23 at 9:45 AM. LPA met with Wendy Peel, Facility Representative (FR).

There were 55 children and 17 staff present when LPA arrived. Facility capacity is in compliance with license and Title Five ratio guidelines. All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District.

LPA toured Classrooms #1/PCC, #2A/B, and #3, #4 and #5. Classroom #4 and #5 are in a detached building. All classrooms had furniture in good condition, free of loose, sharp and/or pointed parts. The floors and surfaces in the classrooms were clean and safe. Rugs were observed to be flat on the ground to avoid tripping hazards. Water is made readily available via operable water fountains. All rooms have water bottles available for children if needed. LPA observed trash can in Room #1 had solid waste without tight fitting lid. Children have cubbies to store personal belongings separate from each other. Children have cots to nap. Bedding is provided by facility and washed weekly by laundry service. There is extra bedding on hand to change as needed.

LPA toured the children’s restrooms located outside Rooms #1 and 2, inside Room#3 and in the detached building connecting Rooms #4 and 5. Restrooms were observed to be safe and sanitary with operable sinks and toilets.

LPA toured the outdoor play area. Outdoor area was observed to have age appropriate toys and material for children, free of loose, sharp, and/or pointed parts. LPA observed required cushioning under climbing structure to absorb fall. Shade was observed throughout the outdoor area and water was observed to be readily available via operable water fountains. LPA observed solid waste in outdoor trash can without a tight fitting lid. ---------------------PAGE 1

LPA observed that facility is still implementing COVID-19 precautions and procedures as required by Los Angeles Unified School District and the Department of Public Health

Incidental Medical Services (IMS):
This facility provides Incidental Medical Services- IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personal, and administrative records. Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

LPA reviewed required posted documentation which should include, Facility License, Publication (PUB) 393- Notification of Parent Rights, Licensing Form (LIC) 610- Facility Disaster Plan, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights and Lunch/Snack Menu. LPA did not observe LIC610 or Lunch and snack menu. LPA advised FR to post documents where parents have access due to parents not entering the facility.

Facility records were reviewed for LIC 9040- Facility Roster, 9148- Earthquake Preparedness form, Daily schedule and Disaster drill log, last drill conducted on 10/20/22. All items observed.

LPA reviewed Sign In/Out sheets located in outside the main office. All children present were signed in with the date, time and full signature of guardian.

Children’s records were reviewed for Emergency Card, Immunization Records, Licensing Form (LIC) 627- Consent for Medical Treatment, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement. Per staff there are currently children with medication. Medication is stored in the office first aid cabinet inaccessible to children. LPA observed required signed documentation for medication. All file reviewed were complete.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. Five out of six staff files reviewed have expired Mandated Reporter Training certificates.

LPA toured the kitchen located across from Room #1. Facility provides AM snack, PM snack and lunch. Food is not cooked at the facility. Prepackaged food is brought from the neighboring, elementary school. Extra food is not stored in the kitchen, it is discarded at the end of the day. Kitchen was observed to be clean, free of litter, insects and rodents. Cleaning supplies are stored separate from the food. Carbon monoxide detector is located in the kitchen. Device was tested and is operable.

During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment. ----------------------PAGE 2
