25 | On 3/22/2024 at 1:00 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management inspection to follow up on multiple incidents that were reported to The Department. The facility reported these incidents to the Department within the required 24 hours and submitted a written report within seven days per Title Regulation requirements. Upon arrival, LPA met with Facility Representative Bertha Morales, Office Manager, who directed LPA to classrooms. Census was taken. Principal was not available during today's inspection due to a LAUSD District wide Principal training.
On 01/04/2024, an incident was reported to The Department via email in regards to facility being broken in and vandalized. Per report, rooms affected were RMs 1, 2, and 3. LPA observed all classrooms and observed that rooms are currently being used at this time. LPA interviewed staff and stated that rooms were inspected by District Maintenance prior to opening for use by children and staff and that some items are still currently being replaced. LPA obtained and reviewed Police Report and photos from incident were provided to LPA during inspection. Staff interviews consistently state that they are confident that all doors were locked by Closing Staff, though no forced entry was observed. Staff states it has not been communicated to them of what District response has been to incident at this time.
On 02/02/2024, an incident was reported to The Department via telephone in regards to allegations reported to Principal by Authorize Representative that her child's personal rights were violated. LPA interviewed Teacher named in allegation (A1), and A1 states that child was having a difficult time during drop off with Authorized Representative and she came to assist. During that time, child tried to run out of classroom and she had to react quickly to keep them from running out holding onto child briefly and then instructing A3 to shut the door quickly. A1 then states she guided child back in using her arms and body in appropriate manner. A1 states teachers in classroom (A2 and A3) witnessed incident. A2 and A3 interviews corroborate A1's statements and stated they did not see the child being handled in a way that would be concerning to them. Per interviews, Authorized Representative was right outside classroom to witness and A2 states she told Authorized Representative that what she was stating occurred is not consistent with what she herself
