25 | On 12/17/24 at 9:25am Licensing Program Analyst (LPA), Claudia Kam made an unannounced case management inspection at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.
LPA met with lead teacher Alyssa Jones Facilty Representative, and was taken on a guided tour of both indoors and outdoors of the facility. Census was taken. During this inspection LPA interviewed witnesses, and staff who reported the incident to Licensing. Per reporting party she followed LAUSD protocols in calling Law Enforcement Agency (LAPD), submitted SCAR report, and informed LAUSD Operations. LAPD notified Facility Representative to handle Administratively, and no investigation was conducted by them. Facility Representative, confirmed on 11/22/24 that no internal investigation will be conducted. LPA confirmed that child is still attending care. Based on the information obtained, there was no violation of personal rights at the facility at this time though there is pending information. LPA requested Facility Representative to provide update to Department should one become available.
Due to insufficient information being available at this time, a follow up visit will be required at a later date.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 12/17/24.
A notice of site visit was given and must remain posted for 30 days.
