Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced annual random inspection. LPA met with Akilah Gipson, Assistant Director, who guided analyst on a tour of the facility. This is a preschool program which consists of 4 classrooms; Purple Room, Blue Room, Green Room and Orange Room. Facility operation hours are Monday through Friday from 7:00 AM to 5:45 PM.

All areas identified were inspected. Upon arrival, the following staff were present during this inspection: Green Room: Staff #1 and #2 with 13 preschoolers; Blue Room: Staff #3 and #4 with 13 preschoolers; Orange Room: Staff #5 and #6 with 15 preschoolers and Purple Room: Staff #7 and 8 with 13 preschoolers. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are provided by representative and washed weekly by the facility. Napping equipment (mats) were observed to be stored in one of the four classrooms in each room. Per Assistant, the isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms and each child has their own cup to take outdoors during outdoor activity time. LPA observed all classrooms have a back door that remain unlocked and are accessible to the children in care. The back door leads to a driveway which is used for automobiles. The door is located next to kitchen area in each classroom. LPA advised Director Assistant to add a sensor mechanism onto each door that will make a sound to alert teachers if and when door may come open for the safety of the children in care.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Assistant Director states that there are no poisons stored at this facility. CONTINUES PAGE 1 OF 3

Immunizations-a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment was inspected safe conditions, free of sharp, loose or pointed parts. LPA observed one tricycle and one large wagon to be missing rubber handles for children to hold onto. LPA advised the Assistant Director to have the two toys removed until the rubber handles can be replaced. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA noticed that the outdoor yard has an area with loose water hoses. LPA advised Assistant Director to be cautious as to how the watering hoses are being stored to prevent a possible tripping hazard. All areas around or under high climbing and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPA issued the Children’s Record Review (LIC 857) and the LIC 859 Review of Staff Records to the Assistant Director during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection. Assistant Director/Teacher Staff #3 is missing Immunization (MMR) this poses a health and safety risk to children in care.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Children's roster was not reviewed. Sign-In and Sign-Out sheets were reviewed.

Disaster drill log was available for review during inspection. Last drill conducted 03/2019.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks are provided by child's representative. Preschoolers are provided with a breakfast, lunch, only.

First Aid supplies were observed in each classroom. LPA observed that the first-aid kit supplies are missing tweezers. According to the Assistant Director, no medication is administered to any children at this time.

This facility does not provide Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the Assistant Director to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA received emails from Assistant Director and Front Office Administrator Assistant to be added to CCLD Advocacy Program's website to receive CCLD's regulations and Quarterly updates.

The following deficiencies listed on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Akilah Gipson, Assistant Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
