On 10/17/2024 at 9:20 a.m., Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced visit for the purpose of conducting a required 3-year inspection. During the inspection, LPA met with Akilah Bailey, Director, and advised them of the purpose of the inspection. A tour of the facility was provided inside and out. LPA observed proper care and supervision during this inspection. LPA observed 64 children indoors with proper teacher/child ratios. There was a total of 10 staff at the time of the visit. Facility hours of operation are Monday- Friday from 6:30 a.m. -5:30p.m. LPA observed all postings requirements for operation on the postings board LIC203A- License, LIC610A-Emergency Disaster Plan, LIC9148- Earthquake Preparedness Checklist, PUB394-Notification of Parents Rights Poster, LIC613A- Personal Rights, PUB-269- Car Seat Law, Menus, Activity Schedule.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are kept in each classroom made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 degrees F or less. Solid waste storage containers have tight-fitting covers and are in good repair.

Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. All materials and surfaces accessible to children are toxic free. The facility is free of flies, insects, and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the childcare center director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department. All children are under supervision, including visual supervision, of a teacher always. LPA reviewed a sample of children’s files and noticed in the files that there was no California Pre- Kindergarten and School Immunization Record in files. LPA Whitmore read the regulation 101220.1(g). The Director explained to LPA Whitmore that the facility uses PRO-CARE App. The immunizations are documented on the App. LPA Whitmore reviewed the staff files two files were missing the Immunization Record. All staff mandated reporter was current.Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have to received a criminal record clearance or exemption. No individuals excluded by the Department can be present. LPA Whitmore reviewed the Guardian List to make sure staff were associated to the facility. There were three staff that were not associated in Guardian. The Director explained to LPA Whitmore that the form the facility uses is the State of California Request for Live Scan Service Form. LPA Whitmore reviwed the forms. LPA Whitmore informed the Director that the facility can no longer use the form effective as of today. LPA Whitmore printed out and gave the Director the correct form to be used which is LIC 9163 Request for Live Scan Service Community Care Licensing. Also, that staff will not be able to return to the facility until their fingerprints are cleared, proper form used, and in Guardian.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep as an additional resource. LPA also informed xxxx of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


A notice of site visit was given and must remain posted for 30 days. The deficiencies on the following were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809- D
