25 | Licensing Program Analyst (LPA) Tiffanie Tran arrived at the above facility to conduct a Case Management Incident inspection to follow up on the self-reported incident that occurred at Childtime Children's Center on 07/18/19. The Monterey Park SW Regional Office received the incident report on 07/26/19. Upon arrival, LPA met with Director. LPA observed proper care and supervision during children's napping time. All center staff that was present during today’s inspection had fingerprint cleared and associated to the designated license number.
Based on the information that were gathered through interview, it revealed that, about 3:55PM during afternoon outdoor play, there were one teacher supervised 10 children. Staff observed C1 was playing at the sand box area with his friends. Children were excited running around looking for another shovel. Staff observed C1 tripped on his footing and landed on the sand area. Child got up and continue playing with peers until child got pick up. The next day, mother brought C1 to school with a splint on stated that child had fractured his right arm. Per staff, child did not show any side of discomfort after he falls. At this time based on the available information it does not appear this incident was the result of a Title 22 violation for lack of care and supervision. No deficiency was cited.
