On 6/9/21 Licensing Program Analyst (LPA) Jeanette Estrada conducted an uannounced complaint inspection to the above facility. LPA met with Ana Fragoso-Tovalin- Director, and went over the findings. At the time of arrival there were 48 children present with 4 staff.
LPA toured the facility on 5/4/21 and observed children eating indoors, not on playground tables. LPA conducted staff interviews which provided conflicting information. The complainant was not available for interview. Based on interviews conducted and the information obtained, the above allegation is unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore these allegations are unsubstantiated. Exit interview was conducted with Ana Fragoso, Director.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
