25 | Licensing Program Analyst (LPA) conducted a case management inspection due to an incident that occurred on 08/30/21. LPA met with Rubina Chadha, Assistant Director, who guided LPA to the office. LPA took a census of the children at approximately 09:30 AM. Administrator, Mala Chabra, arrived during this inspection.
The incident that occurred on Monday 08/30/21 (Incident #1), was reported to the Department on Tuesday, 08/31/21 via email. The incident was reported to the department within 24 hours. During this inspection the Administrator disclosed to LPA that 02 other incidents were observed after viewing the footage from 08/30/21. Footage was initially observed on 09/01/21 becoming aware of the incidents on 09/01/21. LPA completed incident reports for these incidents during the inspection.
LPA Mora conducted staff interviews, obtained documentation and observed video footage from 08/30/21 during this visit. LPA was sent video footage obtained.
Based on the information provided during this visit, it was found that S1 (Staff #1) bopped a child on the head and face several times with a cushion pillow due to the child not listening. It was then observed that S3 (Staff #3) pulled the same child's ear across the room. Furthermore, it was observed on the video footage that prior to these two incidents occurring, approximately 4-5 minutes before, S1 pulled child #2's hair in an attempt to sit the child down. S1 then grabbed the child by the arm to forcefully sit the child down. The three incidents which occurred are a violation of each child's personal rights. Each child in care shall be accorded the following: to be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. These incidents were an immediate risk to the health and safety of children in care.
