25 | Licensing Program Analyst (LPA) Staicy Perry conducted a case management inspection due to an unusual incident that was reported to the Department on 9/10/2024. LPA met with Mina Rodriguez, Lead Teacher . Census was taken and LPA observed 0 children.
On 9/9/2024 in the morning the facility became aware of HVAC system to not be functioning. Facility informed parents to pick up children due to repairs needed on HVAC system. Facility resumed normal operation the following day 9/10/2024. During the inspection, heating, lighting, and ventilation was evaluated by LPA Perry. LPA observed central AC/heater. LPA observed the thermostat located in classroom and temperature to be at 70 degrees. The facility reported the incident to the department within the required time frame. LPA obtained declaration from Mina and other documents.
Based on the information obtained on this date, no follow-up is necessary regarding the incident. The incident appears to be an unusual accident. It appears to be nothing the facility staff could have done to prevent the incident from occurring.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
