25 | An unannounced case management inspection was conducted by Licensing Program Analyst (LPA) Staicy Perry on Monday, April 28, 2025, in response to an Unusual Incident Report (UIR) submitted to the Department on March 7, 2025. The UIR referenced an incident that was alleged to have occurred on or around March 6, 2025.
The incident reported involved Child #1’s mother alleging that S#1 had pulled child’s #1’s ear and told child #1 to go to sleep. When the FSS, Family Services specialist, Adriana Villasano attempted to obtain more details from the parent, the mother stated that she requested that her child be transferred to another licensed childcare facility under the same provider. Per Child Development Specialist ,Patty, the child was successfully transferred to another licensed site, and no further concerns or issues have been reported by the parent or regarding the child.
During the inspection, LPA Perry conducted the following: Interviews with facility staff, collection and review of written staff declarations, observations of facility operations and interactions between staff and children. LPA did not observe any violations of personal rights or inappropriate treatment of children during the visit. Additionally, there was no evidence gathered through staff interviews or facility observations to support the allegation.
LPA attempted to contact the parent of Child #1 and left voicemail's; however, no return call was received as of the time of the inspection.
Based on interviews, declarations, and observations conducted during the investigation, there is no evidence to substantiate the allegation. No personal rights violations were observed or determined. LPA provided the facility with a copy of Title 22 regulations concerning Children’s Personal Rights for reference.
This incident has been investigated, and no deficiencies were cited under Title 22 Regulations.
An exit interview was conducted and a copy if this report along with Appeal Rights was provided to the Child Development Specialist, Patty Linares. A Notice of Site Visit was provided and must be posted for 30 days.
