Licensing Program Analysts (LPAs), Nathanael Mooberry and Melissa Zaragosa, conducted an unannounced case management visit on 03/12/2026. LPAs arrived at 10:44AM. LPAs met with Rosanna Astraquillo, Principal, who provided a tour of the facility. There were 112 children with 21 staff present. The purpose of the visit was to follow up on two unusual incidents, one reported to have occurred on 02/23/2026 and the other reported to have occurred on 02/24/2026.

Licensee serves preschool children ages two years through five years of age. The preschool program consists of five classrooms: PCC Room, Room 2, Room 3, Room 4 and Room 5. During a walk through of the classrooms, the following staffing ratios were observed: PCC Room: 19 children with 5 staff present; Room 2: 27 children with 5 staff present; Room 3: 21 children with 4 staff present; Room 4: 22 children with 4 staff present; and Room 5: 23 children with 3 staff present. All staff present have Criminal record clearance through Los Angeles Unified School District.

The incident that occurred on 02/23/2026, was reported to the department on 02/24/2026, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.

Per Principal, On 02/23/2026 at approximately 3:45pm there was an unusual incident report. The unusual incident reports involves C1, S1(teacher), S2, (teacher aid), S3(teacher aid), and S4(building and grounds staff). Per Principal, Room 3 was about to transition to Room 1 when C1, according to S1, stood up and pretended to punch and kick the air, and fell back into the into the shelve, hit the back of their head, causing an open wound, and a bit of bleeding.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Per principal, they were informed about the incident and arrived to the class when S1 was placing an ice pack on the back of C1s head. Per principal, C1's parents were contacted and were advised to take C1 to the hospital. Per princpal, bleeding stopped upon C1s father arrival. Per principal, the director was also contacted, a First Aid report was created, an Unusual Incident Report was sent to the department, and an Istar and head injury report was created.

Per director, the child returned to school on 02/26/2026, as requested by the doctor. Per principal, C1 was allowed to return to school with no restrictions or limitations. Per principal, they were provided with a copy of the doctors report, by C1s mother. LPA was provided with a copy of a doctors notes.

The incident that occurred on 02/24/2026, was reported to the Department on 02/24/2026, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.

Information reported to the Department indicated that the school had been evacuated due to an emergency in the surrounding area.

During the visit, LPAs collected a copy of the facility roster, conducted interviews, and obtained other pertinent information.

LPA conducted interviews with Principal. Principal stated they received a call at 6:30AM on 02/24/2026, from Los Angeles Unified School District (LAUSD) Operations advising not to access the school due to an emergency in the surrounding area. At 6:45 AM, LAUSD Operations directed Principal to the command post at Tarzana Elementary. Tarzana Elementary opened up Room 9 and the room next to it for Vanalden Avenue Elementary EEC. The neighboring room was only used for nap time. Principal contacted families and staff through BlackBoard Connect (BBC) to have children dropped off and picked up from Tarzana Elementary. Per Principal, there were 35 children in attendance on 02/24/2026.

During the interview with Principal, Principal stated that 5 PCC students were picked up early and did not nap. Principal stated that Food was provided by Tarzana Elementary. neighboring Early Education Centers dropped off 40 cots and sheets.

Principal stated they met with LAUSD OPerations at 1:00 PM and explained incident to staff. Tey were informed that LAPD had cleared cleared the area at approximately 11:30 AM. Princiapal was told that the plan was to finish out the day at Tarzana Elementary. The next day, Vanalden Avenue Elementary EEC re-opened.

Principal stated that no parents contacted the school with any concerns and they received only positive feedback.

Per Drill logs, last Lock Down Drill prior to incident was 09/26/2025, last fire drill prior to occurance was 02/06/2026, and last earthquake drill prior to occurrance was 02/13/2026. Another lock down drill was conducted on 02/27/2026.

After the incident, Principal gave direction to staff to communicate with the Principal to avoid confusion. Principal plans to have a First Aid Kit in her or another staff's car in case of similar future occurances.

LPAs advised Principal to have a meeting with staff to enhance clarity on procedures and identify and address any unknown concerns.

LPAs advised to determine off site relocation site and update Emergency Disaster Plan.

There were no deficiencies cited during today’s inspection.

Principal was provided with Notice of Site Visit. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Rosanna Astraquillo, Principal, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
