25 | On 12/06/2019 at 10:40 a.m., Licensing Program Analysts (LPA's) Dalicia Adkins and Antonio Almanza arrived at SYLVAN PARK EARLY EDUCATION CENTER – 191890476 to conduct an un-announced case management visit to follow up on an Unusual Incident that occurred at the facility on 11/21/2019. LPAs met with Director Claudia Araujo who guided LPAs on a tour of the facility. LPA’s reviewed Children and Staff files. LPA observed 22 children and four staff in room (1A) and children roster.
The department received a self reported unusual incident report on 11/21/2019 regarding lack of supervision of children in care. It was self reported by Director: after conducting a school fire drill on 11/21/2019 Child #1 was left alone in the classroom (1A) without supervision. Absence of supervision-including a child left unattended, a child supervised only by a person under 18 years of age, or child wandering away due to lack of supervision is a violation of title 22 regulations.
During the inspection, LPA's conducted interviews with facility staff (staff 1-4) involved in the incident and Child#1. LPA's also reviewed child’s record, personnel records and were supplied with copies of other supporting documents. LPA’s reviewed and discussed emergency plan handbook policy and procedures. Reviewed and discussed sign in/out sheets, health check roster, and classroom roster. Based Today’s observations, interviews and supportive documents it was determined that the unusual incident report requires further investigation.
LPA’s collected the following documents: Policy and Procedures, Staff Handbook, Drill Schedule, Emergency Drill Schedule
An exit interview was conducted, and a copy of this report was furnished to Principal Claudia Araujo for review. Notice of site visit was furnished for review and posting.
