On 3/15/2023, Licensing Program Analyst Andrea Pittman met with Lead Teacher Trina Stewart for PS2 and Lead Teacher Olga Lambert for PS1, later Coordinator Christy Depasquale completed the visit. An annual random inspection was conducted. LPA toured and inspected the facility in accordance with the facility sketch. During the inspection LPA observed 18 children and 8 Staff providing care and supervision. Facility Operates in two rooms referred to as PS1 and PS2.
Facility is located on an Elementary school, there are a total of two rooms: two classrooms and one play area utilized for outdoor play.
Operating hours are Monday through Friday from 8:00 AM - 11:00 AM for the first session and 12:30 PM to 3:30 PM for the second session.
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. LPA observed individual storage for children's belongings. The classroom consists of doubled filtered water stored in igloo containers with individual one time use disposable cups for children. An isolation area was inspected located at a desk in the far side of the room unless ECE nurse says symptoms require isolation in the wellness room which is on the Elementary’s campus. Children are not provided a napping time but are provided quiet time, if needed. Age-appropriate sinks and toilets were inspected for availability and good repair. Toilets flushed properly; toilet and sinks are reachable by the children. Each restroom has adequate toilet paper, hand soap, and paper towels available. Restrooms are cleaned, restocked with toiletries, and sanitized by on site staff.

First Aid supplies, smoke detectors, carbon monoxide and fire extinguishers were observed and in operable condition; first aid supplies were stored inaccessible to children in care. Smoke and carbon monoxide detectors tested operational according to fire marshal standards. Trash cans with tight lids were observed but there were also trash cans without tight-fitted lids which staff moved to make inaccessible to children in care. Additionally, there were trash cans with lids that were not on the trash cans; and Staff corrected the deficiency by placing the tight fitted lids on the trash cans. Cleaning supplies are out of reach of children and locked under the sink in PS2 and over the sink in PS1.

Emergency Drills are conducted at least every 6 months, the last drills was conducted and documented on 3/14/2023 at 8:15am for the Fire Drill and on 10/27/2022 at 10:27am for the Emergency/Earthquake Drill. The fire extinguisher (3A40BC) is reading in green and meets the Fire Marshal codes and standards (last serviced 6/17/2022). Food allergies are noted and are posted on a pink sheet on the fridge in each classroom. There is no outside food allowed and the Elementary provides breakfast and lunch, no snacks.

The outdoor play area was inspected and observed to have potential hazards of a water logged flower bed, missing wood chips cushioning the bottom of the slide, and a tricycle with a broken handle; the Staff corrected the deficiencies during the visit by emptying the water bed, repositioning the wood chips, and removing the tricycle. Equipment was inspected for safety, cushioning material, good repair, and age appropriateness.

The playground is well fenced all around, and no bodies of water observed in the outdoor play area after emptying of the flower bed.

Facility maintains adequate teacher child ratio. Care and supervision were evaluated and determine basic needs of children are appropriate and are being met. The parent board was reviewed and has all the required forms posted accessible to parents.


Children's records were reviewed for completeness. Health History, Emergency Contact and Medical Exams; Immunization Records are all in the children's file. The facility roster was up to date and all staff have been fingerprinted and association to the designated license number.
Director and teachers are currently certified in Pediatric CPR/First Aid, expires until 4/6/2023 and 12/24/2023. Child Care Providers (AB1207) Mandated Reporter training certificate is effective until 7/18/2024 and 8/8/2024.

The following Incidental Medical Services (IMS) were discussed.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from date of employment to complete training as required.
