An Annual Random Inspection was conducted by Licensing Program Analyst (LPA) Jennifer Hua, LPA met with licensee Mary Meyers, who guided the analyst on a tour of the facility on this date. LPA observed 13 children in care, 3 infants and 10 preschoolers supervised by licensee and assistant. This is a single story home. Per licensee, residing in facility are 2 adults and 0 minors. The home was inspected for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. Day Care days and hours are: M-F 5:00 AM- 6 PM.

Areas used by children were inspected as follows: Living room, 1 bedroom, 1 bath room, play room in back of home, and fenced back yard.

Per licensee, there are no weapons, firearms, swimming pool or spa on the premises. The backyard is adequately fenced. There are age appropriate toys and equipment on the premises. The smoke detectors, carbon monoxide and fire extinguisher (2A 10BC) are in operable condition. Fire extinguisher was serviced on 2/15/19.

Licensee and assistant are current in Pediatric CPR/First Aid. Certificates will expire on 2/1/21.

Areas off limits include: Garage, kitchen w/dinning, front yard. 2 Bedrooms & bath room

Child Care Roster, Disaster Plan, Emergency Disaster Drill and Children's Records were reviewed. Children records and required licensing forms were discussed as well as mandated child abuse reporting and criminal records clearance (finger prints and child abuse clearance) requirement
No smoking, No infant walkers, Johnny jumpers, exersaucers, bouncers and any other item that falls into that category, earthquake – fire, disaster drills and safety, posting requirements, children records requirements, mandated child abuse and injury/ death reporting, criminal records, child abuse clearance and criminal records transfer requirements, SIDS, Never Shake A Baby, A Child Care Provider's Guide to Safe Sleep was discussed and provided to licensee. Drills conducted on 6/3/19. Child Care policy in place. Licensee and assistant have completed the mandated reporter training, certificate observed.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. · Dog(s) and or pets should be isolated from children in care.
· It is recommended that a First Aid kit be available on premises.
Outdoor supervision required at all times. If outdoor area not adequately fenced provider must be with children at all times when outdoors.

Deficiency cited on attached 809D. Exit interview was conducted with licensee. Notice of Site Visit notice posted. And Licensee advised to keep notice posted for 30 days or a civil penalty of $100 will be assessed.

Immunizations. mmunizations. The licensee shall document each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled.

Upon receipt of this report documenting a substantiated complaint allegation and a Type A deficiency, the licensee shall do the following:
1. Post the Notice of Site visit and any licensing report documenting a Type “A” deficiency.
2. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
3. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
4. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this visit.
