Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced Random/ Annual inspection. LPA met with Valerie Lopez, Director who guided analyst on tour of facility. This is a preschool program which consists of 3 classrooms; RM 1, RM 2 and RM 3. Room 3 is currently not being used due to summer enrollment. Room 3 will reopen on August 19 when the new school year starts. There is an school-age program on the premises with facility number #198007364. Facility operation hours are from Monday to Friday from 6:30 AM to 6:00 PM.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: Staff #1, #2 and #3 with 19 preschoolers. During inspection, all children and staff were outside. The following was observed during tour of facility:

PHYSICAL PLANT

Disinfectants, cleaning solutions, medication and other items that may pose a danger to children, were observed to be inaccessible. Director states that poisons are locked under the kitchen sink. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing sinks are safe, sanitary and are operating properly. All floors are clean and safe.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed to be free of flies, other insects and rodents.

Sign In and Sign Out
The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement was not met as evidenced during review of sign in and out sheets. It shows that only 13 of 19 children were sign in today.

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Staff washes linens and bedding on Friday.

Children's roster was reviewed and is current. Disaster drill log was available, last drill was conducted on 07/23/2019. Teacher child ratios were observed and staff names recorded. Sign in and out sheets were reviewed and was observed by LPA that 13 out of 19 children signed in.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

FACILITY RECORDS
All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence is on file. Educational background, training, and/or experience for each staff present are on file and were reviewed.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Snack menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Children enrolled bring their own lunch. Or Facility provides lunch for the children through Good and Tasty Food Services. The facility provides AM and PM snack.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Based on this information, the following deficiencies listed on the attached LIC 809d are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Valerie Lopez, Director
