CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 01/10/2022 due to licensee is requesting to change the Toddler Option classroom from classroom #4 to classroom #3. LPA arrived at the facility at 8:40 AM and met with Liouba Gasparian, Licensee who guided analyst on a tour of the facility. During this inspection there was 1 toddler and 27 preschoolers present in the facility with four staff. During this inspection licensee, Robert Gasparian also arrived to the facility.

During this inspection LPA Eivazian measured classroom#3, which is located by the entrance hallway in the west wing of the facility and entire facility outdoor playground. Based on facility sketch there are 3 toilets and 3 sinks on the facility west wing and three toilets and 3 sinks on the facility east wing.

LPA observed there were two safety gates installed to the room#3 to make it separated from preschool classrooms. Also a proratable gate was observed in room#3 by the entrance door which leads to the hallway.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the Licensee, Liouba Gasparian at 11:00 a.m..
