An unannounced Required – 1 Year Inspection was conducted on 04/15/2022 at 9:30 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 120 with 41currently enrolled. Licensing staff met with Fabiola Martinez, Director. The program currently operates Monday thru Friday from 6:30 AM to 5:00 PM. LPA toured the facility of both indoors and outdoors. Upon entering facility, LPA filled out COVID-19 questionnaire and temperature check was conducted.

At the initial start of the site visit, there is a total census of 24 children and a total census of nine teachers present. The facility is made up of four classrooms: Eagles Room (3-5 year olds) LPA observed seven children with three staff (S1, S2, S3), Sharks Room (3-5 year olds) LPA observed nine children with three staff (S4, S5, S6) Llamas Room (2-3 year olds) LPA observed five children with two staff (S7, S8), and Bunnies Room (2-3 year olds) LPA observed four children with one staff (S9). LPA observed furniture and equipment were inspected for age appropriateness and good repair in each classroom. Beginning at 9:35 AM, LPA observed each classroom to have child size tables and chairs, learning stations with cubbies for art, science, manipulatives, dramatic play area, and a quiet area with books available. LPA observed toys and other age appropriate materials to be clean and free of hazards at time of inspection. There were cubbies observed as storage for children's belongings in each classroom. Mats/cots were inspected. LPA observed floors of each classroom to be free of hazards and clean. Per Director, linens are provided by the facility and washed at the facility. LPA observed cots stored in corner of classrooms with sheet attached and names written on each cot. Availability of drinking water was reviewed. LPA observed water filtration system in Eagles Room and water pitchers available in each classroom with cups. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. At 9:45 AM, LPA observed Bathroom 1 in between Eagles and Sharks classroom that is shared to have four toilets and three sinks. LPA observed two toilets and one sink to be covered with plastic. four toilets, three low sinks for children and one tall sink within arms reach of diaper changing table. LPA observed one toilet covered by plastic in Bathroom 2. Director states covered toilets and sinks are operable but is being covered to continue to social distancing measures per COVID-19 Health and Safety protocols. Telephone service, heating, lighting and ventilation were evaluated.

At 9:54 am, LPA observed a first aid kit is kept hanging on a wall by entrance in each classroom. Per Director, children requiring medications in Eagles and Sharks classroom is kept in her office. LPA observed Director's office to be accessible through doorway in Eagles Classroom. At 9:56 AM, LPA observed medication stored in a locked bag hanging by First Aid kit in Bunnies Classroom for C6. LPA observed medications to have written consent form with expiration date of 07/2022. Carbon monoxide detectors and smoke detectors are present in the facility. At 9:58 AM, LPA observed fire extinguishers last serviced in 02/22/2021. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items stored in off limits Kitchen.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 9:50 AM, Food preparation areas were toured for safety, cleanliness and proper equipment. LPA observed staff preparing lunch for children at time of inspection. Facility provides lunch and AM/PM Snack for children.

At 10:12 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed two play yards at facility, one for 3-5s and the other for 2-3s, each area is separated by a gate. LPA observed 3-5 play yards to have shaded play area with two play structures with slides and proper cushioning material below that is well maintained. LPA observed shaded sandbox area that is covered by a tarp that Director states is still not currently being used per COVID-19 health and safety protocols. LPA observed concrete and turf grass area through out and area is fully fenced. LPA observed 2-3s play yard to be shaded with a small play structure and proper cushioning material below to absorb falls and fully fenced. LPA observed musical instruments, three small tricycles, and other age appropriate toys well maintained. LPA Chicote advised Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to water jugs during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. CPR card expires in 11/2023 for Director. Inspection of required forms made.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Director:
Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited.

The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 04/15/2022.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
