On 3/23/2023 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at Buford State Preschool located at 10915 S. Felton Avenue, Lennox, CA 90304. LPA was greeted by Christina Hector, Site Director. LPA toured the facility both indoors and outdoors.

Director Nellie Rios- Parra arrived at facility at approximately 11:40 a.m. LPA observed all 5 classrooms, currently the program in only using two classrooms. The following was observed during the initial inspection:
Classroom 1- not being used/ closed for the school year.
Classroom 2- classroom currently being used for program.
Classroom 3- not being used/ closed for the school year due to enrollment.
Classroom 4- not being used/ closed for the school year.
Classroom 5- classroom currently being used for program.

During the initial time of inspection, LPA did not observed any children present in facility. LPA observed 3 staff members. By the end of the inspection, LPA observed 18 children with 3 staff members providing care and supervision. In addition to Site Director C. Hector and Director N. Rios Parra.

Facility operates on a Lennox School district campus. It is physically on the Middle School school site. Days and hours of operation are Monday through Friday 7:50 a.m. .to 10: 50 am and 12 p.m. to 3:00 p.m. Program offer an AM and PM session. Facility is licensed to serve 105 preschool children ages 2 years through entry to First Grade. Facility is open from 7:30 am to 4:00 p.m.



There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.
Medications are made inaccessible and stored in a locked classroom cabinet. Currently facility has two children that requires medication. If medication needs to be refrigerated, medication will be stored in director's office. All necessary paperwork that needs to be included with medication is stored in the classroom with medication and director's office.

Teachers do the day to day cleaning in the classroom. Disinfectants, cleaning solutions and other hazardous items are made inaccessible in locked classroom cabinets. Cabinets were observed to have safety latches making the content inaccessible to the children in care. School district maintenance comes daily after school and mid-day to disinfect classrooms.

Capacity and limitations as specified on the license are being maintained. Currently only 2 out of the 5 classroom are being used. LPA reminded faciltiy that any closures of classrooms need to be reporter to LPA. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin.

Facility provides meals through the school district, Buford Elementary school provides meals for the facility. Menu is posted in each classroom's parent board. Parents also receive menu via Class Dojo online application and school district website. All food is protected against contamination and any contaminated food is discarded immediately. Preschool program coordinates with school cafeteria staff to ensure that all dietary restriction are being adhere too. LPA observed all classrooms have a “Medically fragile list” to ensure all staff is aware of the allergies. Solid waste storage containers have tight-fitting covers and are in good repair.

Drinking water is available both indoors and outdoors. Children bring in their own refillable water container and school will provide water bottles when children forget to bring in their water or run out of water. Children are not using water bubblers in classroom.

Areas around high climbing equipment and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility contracts with a Pest control vendor to ensure that facility stays pest free.

LPA observed working fire extinguishers and smoke detector and carbon monoxide detectors in the each classroom. Facility has a centralized fire alarm system. First Aid kits were observed in the classrooms with allergy list/Medical Fragile list included.

LPA observed the cemented walkway of the facility to have slight cracks. LPA advised site director to start looking at options to ensure that cement does not cause any injury. LPA issued a Technical advisory. Director, N. Rios Parra, confirmed facility will add cautionary signs around the area.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director’s Pediatric CPR and First Aid was completed on 5/14/2022 and Mandated Reporter Training was completed on 1/25/2023. Staff files are also kept in the main office with completed health screenings.
LPA reviewed a sample of children’s files and observed 6 files were complete. Site director stated that some forms are kept in the main office and can be access when requested. LPA provided facility with a copy of the LIC 311A and LIC 125 to use as a reference to ensure all files have current documents.
The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility currently using Care Connect as an electronic sign in/out and hard copy is available for LPA’s review.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative, Christina Hector and Nellie Rios Parra was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently IS being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPA and facility admin discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with the licensee facility representative Christina Hector and Nellie Rios Parra. Facility was provided with a copy of their appeal rights.
