On 11/21/2024 at 7:30 AM, Silva Garibyan, Licensing Program Analyst (LPA), conducted an unannounced Required - 3 Year Inspection. LPA met with Liliya Chulpaev, Director and toured the facility indoors and outdoors. Days and hours of operation are Monday through Thursday from 8:00 AM to 4:00 PM and Fridays 8:00 AM to 1:00 PM. The facility consists of 2 buildings with a total of 5 classrooms. The North building (6021) has 3 classrooms and the South building (6017) consists of 2 classrooms. The facility is located on the same premises that Or Hachaim Academy Private School (grates 4-8, girls only) is located in. The private school is on the second and third floors of the North building. There is a unarmed security guard at the entrance of the school. The facility is currently using the classrooms (K1, K2, K3) in the North Building and the playground in the North Building. There are working telephones, first aid kits, and smoke/carbon monoxide detectors in the hall way. There are pull down fire alarms and fire extinguishers (serviced 09/17/2024) located in the hall way, by Director's office. Upon arriving at the center, LPA observed 20 children being supervised by 3 teachers. LPA observed 11 children in Room K2 (4-5 year olds) supervised by two teachers and 9 children in Room K3 (2 -3 year olds) supervised by one teacher. Facility is operating within capacity limitations. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. LPA reviewed the sign in and sign out sheet to verify the census. There are no bodies of water on the premises during the visit. Disaster drill log was available during today’s inspection, last disaster drill was conducted on September 12, 2024. Per Director, isolation area for sick children is in the office. There is no swimming pool or other bodies of water on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. Page 1

Fixtures, furniture and equipment are adequate and well maintained. LPA observed the following postings by the office: License, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269). Furniture and equipment were inspected for age appropriateness and good repair. The classrooms were inspected to ensure that the floors have a surface that is safe and clean.

A walk through of the classrooms was conducted, classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate.

There is adequate heating, lighting and ventilation. Drinking water is readily available in each classroom. Drinking water is available both indoors and outdoors via personal water bottles. There are first aid supplies in each classroom and in the office. .

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. LPA observed mats available for each child. Mats are being sanitized once a week. Bedding is stored separately. Napping children are properly supervised within ratios. Isolation area for sick students is located in the office. LPA observed adequate arrangements for isolation and care of ill children.



LPA inspected and observed a total of 2 clean bathrooms in the North Building. There are a total of two toilets, three urinals, and three sinks available for children's use. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate. Bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom areas.
Children are inspected for illnesses as they arrive. Medication and Discipline policies were discussed. Page 2

The facility provides snacks only. Menus were observed. The food preparation area is by the door leading to the playground. Sharp utensils are stored upstairs, inaccessible to children. Opened snack items are properly stored in air tight containers in the cabinet by Room K3. Trash cans used for solid waste disposal contain tight fitting lids. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food.. The preschool maintains a list of children with allergies and a copy is also kept in the child's classroom.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Fencing around the perimeter of the play area is at least 4 feet high. Play area was inspected for hazards and inaccessibility to bodies of water. The facility is free of flies, insects and rodents.

LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights on file. In all children's records reviewed were missing Physicians report. The children's roster was available and current. Staff Records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits required for Infant Care, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination, Mandated Reporter training. Director and staff did not have proof of a current CPR & First Aid training and Mandated Reporter Training.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
