On 03/10/2020 at 8:30 AM, Licensing Program Analyst, Stella Gutierrez conducted an unannounced annual random inspection at the facility. LPA met with Ericka Polland, Infant Teacher. LPA, Gutierrez discussed the purpose of the visit and provided LPA a tour of the facility inside and out.

WIZ Child Care Center consists of 2 buildings. 119 W. Arbor Vitae, Inglewood, CA 90301 is the infant center building is located behind gates and the other building is located on the main street (40 MPH but during school hours a speed limit of 25 MPH) 121 West Arbor Vitae Street, Inglewood CA 90301 that serves ages 2-6 and 6-12. Operating hours are 6:00 AM-6:00 PM Monday – Friday and ages served are Infants, 0-24 months and a dual license for other building #197403701 that serves ages 2-6 years/ 197403701 that serves 6-12 years.

Inspection conducted today of the Infant Center. Upon arrival LPA observed 1 staff providing supervision and care for 5 infants. The facility was not operating within ratio in accordance to title 22 regulations. Facility is operating within capacity of the license. Staff observed today have a criminal record clearance and is associated to the facility.
There are no pools, spas, or any small bodies of water on the premises. There are no weapons or firearms kept on the premises. There is adequate heating, lighting and ventilation. LPA observed the classroom to have age appropriate material and equipment. Furniture in the classroom is found to be in good repair and age appropriate with sufficient space for physical activities for the children. Napping equipment and beddings are inspected for good condition, appropriate storage and cleanliness. Changing table, napping equipment placement, bedding identification (8 pack n plays) separate napping area and storage areas were observed and inspected. Isolation area for sick children is provided in front office of infant Center Changing station was inspected, Two Infant changing tables that have a padded surface no less than one inch thick and is a washable vinyl plastic material and is raised sides of at least 3 inches. Infant changing station was sanitary and provided with NO arm’s length accessible sink. Sanitize wipes and gloves observed for infant changing area. Page 1 of 6

Children and staff records are reviewed during the inspection. First Aid supplies and medications (locked in file cabinet) are kept in the Administrator's office. LPA observed a working smoke alarm and carbon monoxide located in main care area. Required postings were observed today, including the Facility License, Parents’ Rights Poster, Car Seat Laws and Emergency Disaster Plan LPA observed adequate toilets and sinks to accommodate the facility’s capacity. LPA observed 5 high chairs for infants to use while eating. Refrigerator was inspected. No expired products and food was in storage containers labeled. Toilets flush properly, toilet and sinks are reachable by the children in care. The restroom has adequate toilet paper and paper towels available. The bathrooms were found to be clean and free from hazards.

Outdoor space was inspected today. Outdoor yard is accessed through back door of pre-school classroom. There is appropriate fencing around the perimeter of the outdoor play area. The surface of outdoor activity space is maintained in a safe condition and is free of hazards. All storage containers for solid waste, including moveable bins, have tight fitting covers that are kept on, and are in good repair. The outdoor space is cushioned with material that absorbs falls. The Administrator takes measures to keep the facility free of flies, other insects.

SIDS & SHAKEN BABY SYNDROME INFORMATION (discussed) flyer provided.


Licensee was reminded that all infants must be placed on their backs when sleeping. LPA provided safe sleep flyer and discussed safe sleep practice. Infants must be placed on their backs and must be physically checked every 15 minutes to gauge temperature and ensure they are breathing to prevent S.I.D.S. (Sudden Infant Death Syndrome), and that the Provider’s are required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome. JERRY HALE , Administrator stated that she understands safe sleep practice and prevention of Shaken Baby Syndrome for infants and to ensure supervision at all times infants are in care at WIZ CHILD CENTER INFANT facility. Administration:

Mandatory Forms for the children’s files and staff’s files, Requirements for disaster drills and documentation for both. Roles and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov. Licensee is made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee is advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.

The ADA requires that child care providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services


Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12-month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB393) poster must be posted in an area accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

The facility currently provides IMS. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The licensee was informed of The Child Care Advocate Program (CCAP) that is available from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: www.mandatedreporterca.com

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.


New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. Appeal may be submitted and mailed to the following address:

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

Administrator was informed of (TSP) and will let LPA know if this is something the facility is interested in at a later time.

The facility administrator currently receives queartley updates from CCLD/Child care advocate updates to ensure compliance with Title 22 regulations changes that may occur.

Upon receipt of a citation for a Type A Deficiency, the director shall post the licensing report for 30 days in addition to the Notice of Site Visit & provide copies of the licensing report (LIC 809D/9099D) to parents/guardians of children in care at the facility by the close of business the following day or the next day child returns to the facility. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file.

The facility is found not to be in substantial compliance in accordance with Title 22 regulations and Health and Safety Code standards. There will be deficiencies cited during today’s inspection. Please refer to (809-D's)
