Licensing Program Analyst (LPA) Judy Mora conducted an unannounced Annual Required inspection. LPA met with designated facility Site Supervisor, Adriana Hermosillo, who guided LPA on a tour of the facility. This is a preschool age program licensed for 60 children which operates Monday – Friday from 6:30 AM – 6:00 PM. Per the Director there are 54 children enrolled. There is an Administrative File for the facility under Amistad Preschool, license # 198008821.

All areas identified on the facility sketch were inspected. Upon arrival, LPA observed 14 children with 04 staff in Room B and 22 children with 03 staff on the outdoor playground. Teacher-child ratios were observed to be in accordance with Title 22 regulations. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. LPA observed all required forms/publications to be posted and visible to children's authorized representatives. The Licensee is within the conditions, limitations, and capacity specified on the license.

Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean; LPA observed the carpet in Room B is noticeably soiled with several stains through out the space, this is a potential risk to the health and safety of children in care. All toilets and hand washing facilities are in safe conditions, however, LPA observed a trail of ants in the restroom where the changing table is located. LPA observed ants on diapers and the diaper changing table; this is a potential risk to the health and safety of children in care. All materials and surfaces accessible to children are toxic free. At this time, the Site Supervisor's office is used as an isolation area. There are chairs in the office. Parents are contacted immediately when children are determined to be ill. First Aid supplies are available and complete. Sufficient individual storage space for children was observed. Each child has a cubby. LPA observed sufficient napping equipment. Napping equipment is in accordance with Title 22 regulations.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast, lunch and PM snack. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is brought to the facility daily from the International Institute of Los Angeles. All food is protected from contamination. There is drinking water fountain available in all indoor classrooms and water pitchers and disposable cups are taken outdoors. All storage containers for solid waste do not have tight fitting covers on; this is a potential risk to the health and safety of children in care. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children.

Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Areas around and/or under climbing equipment, swings and slides have rubber as a cushioning material to absorb a fall. Director states there are no weapons or firearms on the premises.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff records were not reviewed due to all staff records being kept at IILA main office, a file review for staff will be conducted at a later date. A random sample of Children’s Records were reviewed to ensure that they are complete. Criminal Record Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. During this inspection LPA also addressed an unusual incident report that occurred on 04/22/19. The incident was reported to the Department on 04/23/19. Interviews were conducted with staff. Staff #1 was running to the restroom with child #1, staff #1 tripped and fell, the child also fell on his stomach and left hand. Child #1 hurt his thumb and it was swollen. The child was not accorded healthy and safe accommodations; this was a potential risk to the health and safety of the chid in care.

Buildings and Grounds
The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
The licensee shall take measures to keep the center free of flies, other insects, and rodents.
This requirement was not met as evidenced by LPA observing a trail of ants
in the restroom. Ants were observed on diapers and the changing table. This is a potential risk to the health and safety of children in care.

Fixtures, Furniture, Equipment and Supplies
All containers used for storage of solid wastes, including moveable bins, shall have a tightfitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.
This requirement was not met as evidenced by LPA observing trash cans to not have tight fitting covers on. This is a potential risk to the health and safety of children in care.

Indoor Activity Space

Personal Rights
To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.
This requirement was not met when Staff #1 ran with child #1 to the restroom, resulting in both falling and the child being hurt. This was a risk to the health and safety of the child.

REMINDER: Failure to obtain a criminal record background check clearances prior to initial presence in the facility will result in an immediate $100.00 dollar or more per day Civil Penalty.

The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiencies that are being cited and need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
