25 | On Friday June 14, 2024, Licensing Program Analyst (LPA) Staicy Perry conducted a Case Management inspection to amend the visit report from inspection conducted on 6/11/2024 and to assess civil penalties. LPA Perry met with Supervisor Claudia who guided LPA on a tour of the facility. Census was taken and LPA observed 11 children and 5 staff.
During the inspection conducted on 6/11/2024 the following deficiency was 101229(a)(1) Responsibility for Providing Care and Supervision. No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation, and 101223 Type A Personal Rights states (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful, and comfortable accommodations, furnishings, and equipment to meet his/her needs. During today’s visit civil penalties in the amount of $500 are being assessed for Absence of Supervision due to a child wandering away from the facility on 6/10/2024. On this visit LPA Perry amended the LIC-809D page from the previous visit conducted on 6/11/2024 to include the civil penalties being assessed. LPA Perry also amended page 2 of the report to reflect correct verbiage.
