25 | On 06/04/2024 at 12:15 PM an Unannounced Required – 3 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with Facility Representative who granted entry to facility. Facility is currently licensed for a capacity of 70. The program currently operates Monday thru Friday from 7:00 AM to 6:00 PM. LPA singularly toured facility both indoors and outdoors. LIC 125 was provided. Facility Representative, Monica Chakmian, Assistant Director, arrived after LPA toured facility and Licensee/Director Siranush Chakmian arrived later.
The facility is made up of six classrooms: 2-3 Year Old Room, 3 Year Old Room, 4 Year Old Room, 4-5 Year Old Room, Speech Therapy Room, and a Multi-Purpose Room (upon entry). At the initial start of the site visit, LPA observed children transitioning to nap time with most of the children still awake at time of inspection. LPA observed 22 children with one staff in 2-3 Year Old Room, 12 children with one staff in 3 Year Old Room, 10 children with one staff in 4 Year Old Room, and 17 children with one staff in 4-5 Year Old Room. LPA observed 2-3 Year Old Room and 4-5 Year Old Room operating out of ratio at time of inspection. S3 states that S2 just walked a child out to the parent and S2 would be returning, LPA observed S2 return to 2-3 Year Old Room resulting in a 22 children with two staff ratio. After some time, LPA observed S6 return to 4-5 Year Old Room, resulting in 17 children with two staff. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate and staff names recorded.
Beginning at 12:25 PM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed a variety of age appropriate toys free of loose and sharp parts available and an area for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected and observed laid out. LPA discussed requirement to place cots in a way that it does not hinder walkways and entrances/exits. Per FR, linens are provided by the families and taken home weekly for washing. Availability
of drinking water was reviewed. LPA observed water coolers available for children and children's personal
