On 05/19/2023 Licensing Program Analyst (LPA) Alemoh met with the facility Site Supervisor, Rose Coorough, for the purpose of conducting a Random Annual Inspection for the State Preschool. This Preschool Program is located on the Plainview Elementary Campus. A tour of the facility was conducted. Upon arrival LPA observed 28 preschool children and 4 staff. This program is a full day session from 7:30AM - 4:00 PM. Incidental Medical Services (IMS) policy was discussed.

Furniture and equipment were inspected for age appropriateness and good repair. All rooms(Class 01 and 02) are clean and safe. Telephone service was verified. Heating, lighting, and ventilation are adequate. Children store personal items in there own cubbies. There are 5 cabinets in each classroom that are locked and contain extra storage for play equipment, toys, and arts and crafts. LPA observed age appropriate toys and materials around the classroom. Drinking water is available inside the classrooms in the form of water dispensers and disposable cups. LPA observed cots, and blankets labeled with the child's name. Parents take home the blankets for washing weekly. Meal logs are located in both classrooms. There is a First Aid Kit, Fire Extinguisher, Smoke Detector and a Carbon Monoxide Detector on the premises.

Class 01: LPA observed 13 children and 2 staff members.
Class 02: LPA observed 15 children and 3 staff members. Medicines are stored in the locked bag high enough making it inaccessible to children. LPA observed the diaper changing log to be current and up to date.

LPA inspected and observed two clean bathrooms, for the boys (6 urinals, 5 toilets and 6 sinks) and for the girls bathroom (8 toilets and 7 sinks). Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Additional sink is located in each classroom. Each classroom sink has there cleaning supplies underneath the sink which is locked during business hours. The sinks are utilized for children to be able to wash their hands.

Children are inspected for illnesses (wellness policy) as they arrive. LPA observed IMS policy plan for certain individual children in each classroom. I'll children are placed in the staff office while waiting for pick up.

Outdoor play equipment was inspected for health, safety, cushioning material, good repair and age appropriateness. The play area is mostly concrete, there is a foam padding under the climbing apparatus. The area was observed to be free of debris. There is an area for shade and rest. Play area was inspected and found to be free of hazards and inaccessibility to bodies of water. Large jungle gym is anchored. Water is provided in the form of an igloo cooler and disposable cups.

Facility serves Breakfast, lunch, afternoon snack. Meals and snacks are provided by VOA main office and are refrigerated or kept in a warmer oven until served by a nutrition specialist. The fridge, microwave and warmer are located in the staff room, where there is an area sectioned off for food preparation. All food is labeled and stored properly. Allergies are listed on the fridge.

Sign in and out sheets were reviewed. The parent board was reviewed and has all of the required forms posted. Fire/earthquake drills current. Roster current.

Teacher child ratios were observed and staff name recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate.

Children's records were reviewed. Staff records are kept in the VOA main office. Staff are certified in Pediatric CPR and First Aid expire 08/2023 and the mandated reporter expires 01/06/24
