On 8/24/2022, Licensing Program Analyst (LPA) Lillian Casillas conducted an unannounced Annual Required Inspection for the infant license. LPA met with Licensee, Yasmeen Chunara and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday, 7:15am to 6:00pm.

This facility includes a toddler component. The facility includes 2 activity rooms, 1 napping room, 1 bathroom, and 1 outdoor playground. Activity Room 1 is for the infants and Activity Room 2 is for the toddlers. LPA observed 21 children with 5 staff.

In Activity Room 1, LPA observed 2 children's tables and age-appropriate play equipment. LPA also observed 1 sink and a changing table. The napping room is located directly behind Activity Room 1. In the napping room, LPA observed 6 cribs and 1 sink. In Activity Room 2, LPA observed age-appropriate furniture, and 1 bathroom with 2 toilets, 2 sinks, 1 changing table, and 3 training toilets. The outdoor playground is large and includes age-appropriate equipment that is in safe condition. The outdoor play area features astro-turf and shade is provided by trees.

Each crib and mat is occupied by only one infant or toddler at a time and cribs are kept free from all loose articles, including blankets and pillows and there are no objects hanging above or attached to the crib. Infants are not swaddled while in care. LPA informed Licensee that staff must physically check on sleeping infants every fifteen minutes and document any signs of distress, including flushed skin color, increase in body temperature, restlessness and labored breathing. LPA provided Licensee with the LIC 9227 Individual Infant Sleeping Plan form to complete and maintain in each infants (up to 12 months of age) file. Facility places infants up to 12 months of age on their backs for sleeping. Staff-infant ratio requirements are being met while infants are sleeping.



Licensee states children bring their own meals and facility provides snacks. Snacks are prepared in the kitchen of the preschool license #191601500. [CONTINUE ON PAGE 2]

Capacity and limitations as specified on the license are being maintained. All staff are trained in CPR and Pediatric First Aid. The name of the child care center director or fully-qualified teacher designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

LPA observed a fire extinguisher Activity Room 2 as well as a carbon monoxide detector.

LPA reviewed a sample of children’s files and observed files were complete, except for LIC 9227. LPA reviewed a sample of staff files and observed files were complete.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. Licensee will send proof of LIC 9227 sent to parents as well as template for the 15 minute infant sleep checks by 8/26/2022.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
