On 9/12/2023 Licensing Program Analysts (LPAs) Evelyn Garcia and Isabel Ortega conducted a subsequent investigation inspection to deliver complaint investigation findings. LPA meet with facility Director, who guided LPA on a tour of the facility. Upon arrival LPAs did not observed any school aged children in care, at 2:20 p.m. 6 children arrived. There was a total of one staff providing care and supervision.

During this investigation, LPAs received pertinent documents related to this investigation, which included Facility Roster and other documentation related to the allegation including teachers files. LPAs interviewed the staff, parents, children and completed an observation at the facility. According to interviews conducted, observations completed, and documentation, there were no disclosures children the physical plant is harmful to children. Also, based on interviews conducted two staff are always with children during outdoor play in the school age yard.

Although LPA’s did observe black slime growing in the school aged play yard, it is not considered to be harmful or toxic to the children and staff. Interviews conducted with staff, children and parents did not disclose any children getting ill from this natural organic environmental fungus. To prevent children from staining clothing or shoes staff will ensure that there is no black slime growing in the yard prior to engaging in outdoor play.

Therefore, Allegations -Neglect/Lack of Supervision and Physical Plant are deemed to be unsubstantiated. Finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged allegations occurred.



An exit interview was conducted, a copy of this report, appeal rights and a notice of site visit were provided to facility.
