25 | Licensing Program Analysts (LPAs) Nolan Tcheng and Suzana Safaryan conducted an unannounced Case Management-Incident inspection for the purpose of following up on a incident that was reported to the Department on 10/31/2023. Upon arrival at 1:35pm, LPAs met with Assistant Director Vanessa Ruiz, to whom the purpose of the inspection was explained. Tour of the facility was provided. Children were present during tour.
Census was taken. There were 27 children with 8 staff members. During today's inspection, LPAs conducted interviews with two staff members and obtained documentation in the form of pictures and ouch report.
On 10/26/2023, a child was in the outdoor play area by the sandbox. Child was playing by the sand and was walking on the play area rocks and tripped, landing on their right eyebrow and bridge of nose. Child received immediate first aid on site, and further medical treatment to close the open wounds. Interview with Staff #2 disclosed that they observed the child fall while they were playing outside. Staff #2 was directly in front of the child as they were walking back from grabbing a toy silicone spoon and bucket, and tripped. Per Staff #1, child had toy equipment in both hands as they were walking along the play area rocks. Pictures of the play area where the incident took place, were obtained. LPAs received pictures of the child playing just moments before the incident occurred, on incident date. Ouch report was reviewed.
The Department has reviewed the incident, and has determined that the facility staff could not have done anything to prevent the incident from occurring. Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Assistant Director Vanessa Ruiz, at 2:45pm. Copy of Report provided.
