On 03/10/22, LPA Liana Stepanyan met with Anita Byramian to provide a report regarding subsequent Case Management (inspection) conducted on 02/25/22. However, the report was inadvertently generated under the wrong facility.

On 02/25/2022, Licensing Program Analyst (LPA) Liana Stepanyan met with Anita Byramian who guided LPA on a tour of the facility. Upon arrival, LPA observed 8 children under care and supervision with 4 staff members. The purpose of this visit was to conduct subsequent Case Management (inspection) regarding an unusual incident that occurred at the facility 02/10/2022 and was received at the Department on 02/11/2022.
The unusual incident indicated that on 02/10/2022 during outdoor play time, child #1 was riding a bicycle, the child tripped while getting off the bicycle. Child #1 fell down face first and hit her chin on the concrete floor. First aid measures were taken, and child’s parents were notified and picked up immediately.

On today’s inspection, LPA interviewed children. Based on interviews conducted staff #1 observed the child #1 fall off the bike, however, staff #1 was not in close enough proximity to prevent the fall. LPA observed outdoor play area free from hazardous items that could cause child to fall off the bicycle. Based on the interviews conducted and LPA observation, the incident does not appear to have been the result of any violation of the Title 22 regulation, therefore, no deficiencies were cited.

An exit interview was conducted and copy of this report was provided to Anita Byramian along with notice of site visit.
