On February 19, 2020 at 2:00 PM, Licensing Program Analysts (LPAs) Monique Ayala and Tiffanie Tran conducted an unannounced Required – 1 Year inspection to the facility noted above and met with the Lead Teacher. LPAs were guided on a tour of the facility. This facility is a Los Angeles Unified School District Preschool Program that is located on an Elementary School premises. This facility has two (2) separate sessions that operates for three (3) hours each; Monday to Friday, 8:00 AM to 11:00 AM (breakfast served) and 12:00 PM to 3:00 PM (lunch served).

All areas identified on the Facility Sketch were inspected. At the time of the inspection, one (1) teacher and 2 teacher assistants with 13 preschoolers were present. Teacher and child ratio were observed, and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children are met appropriately.

LPAs observed an approved waiver posted near the license for outdoor play and restroom use. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Napping is not provided due to the program operating at 3-hour increments. Storage for children’s belongings and an isolation area were inspected for availability and good repair. Availability of indoor drinking water was observed.

The facility has a school nurse on site, once a week. Per the Lead Teacher, office staff attend to the children when the school nurse is not available. First aid supplies were inventoried. Disinfectants, cleaning solutions and other items that are dangerous to children were inaccessible to children. The Los Angeles School District provides daily breakfast and lunch. Menus were reviewed for appropriateness to children in care.

Outdoor equipment was inspected for safety, good repair, age appropriateness, and cushioning material that could absorb a fall. The outdoor activity space is maintained in a safe condition, free of hazards, and inaccessible to bodies of water. LPAs observed adequate shade, fencing and availability for outdoor drinking water.

LPAs reviewed 3 children’s records for completeness. LPAs did not review staff records during the inspection due to the staff records being stored at the administrative office. LPA Ayala will review staff records on a later date. There is at least one person trained in Pediatric First Aid/CPR that was present during today’s inspection.

The children’s roster was reviewed and is current. Sign-in and sign-out sheets were reviewed. The disaster drill log was available, and is conducted on a monthly basis.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA Ayala advised the Lead Teacher to access forms, regulations and quarterly updates on the Department website at: www.ccld.ca.gov. LPA Ayala also advised the Lead Teacher of the Mandated Reporter Child Care Training requirement that went into effect on January 1, 2018, and is available at: www.mandatedreporterca.com.

There were no deficiencies cited during today’s inspection. The Notice of Site Visit (LIC 9213) must remain posed for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

An exit interview was conducted and a copy of this report was provided to the Lead Teacher.
