On November, 30, 2022, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced complaint investigation. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Principal, Emily Hooks who guided LPA on a tour of the facility. LPA observed 73 preschool children in care.

Staff did not allow day care child to rest without distraction: Complainant alleged on 11/03/2022, Staff #1 (S1) took Child #1 (C1) to the "steam lab" that is available for children who do not want to nap during nap time. Complainant stated that nap time starts at 12:15pm and C1 was brought to the steam lab around 12:35pm. Complainant stated that S1 stated, C1 was laying on his mat quietly, not disturbing any children but was not sleeping being the reason S1 brought C1 to the steam lab. Complainant was told by Staff #2 (S2) that C1 was crying telling S1 he wanted to go to his bed and nap.

During the investigation LPA interviewed Staff #2 (S2), Staff #3 (S3) ,Staff #4 (S4), Child #1 (C1), Child #2 (C2) and Child #3 (C3). LPA did not interview Staff #1 (S1) due to S1 being absent from the facility.

Based on interviews conducted C1 stated that S1 took C1 to the steam lab during nap time. C1 stated that he told S1 he wanted to nap. S2 stated that C1 was taken to the steam lab but was rubbing his eyes and crying stating he wanted his bed and wanted to take a nap. C1 was taken back to his classroom to nap. S4 stated that S1 sometimes does not give the children enough time to fall asleep and sends the children to the steam too fast (10 minutes from when nap time starts). S4 stated that incidents like this have occurred twice, where children are sent to the steam lab when the child(ren) want to rest. The above allegation is deemed SUBSTANTIATED.

A Type B deficency is being cited today 11/30/2022 in accordance with Title 22 Regulation code, 101223(a)(3), see LIC9099D.

An exit interview was conducted and a copy of this report was provided to principal, Emily Hooks along with a Notice of site visit and Appeal Rights.
