On 11/9/2022, Licensing Program Analyst (LPA) Lillian Casillas conducted an unannounced annual inspection. LPA met with Director, Rachel Graves, who guided LPA on a tour of the facility. Day and hours of operation are Monday through Friday, 8:00AM to 5:00PM.

The facility has 3 classrooms: RM 1: Dolphins (4 - 5 year olds), RM 2: Kangaroos (3 - 4 year olds), and RM 3: Kittens (2 year olds).

LPA observed 35 children in care and 8 teachers present during this inspection. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. LPA reviewed the sign in/out sheet to verify the census.

The following ratios were observed:
RM 1: 13 preschool children with 2 teachers
RM 2: 13 preschool children with 2 teachers
RM 3: 9 preschool children with 3 teachers

A walkthrough of the classrooms was conducted, and classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. LPA observed classroom space to include dramatic play, arts/crafts, reading, individual cubbies for storage and learning activities. The director's office is the designated Isolation area.

Facility offers children bottled water. Director provided a copy of the lead water analysis conducted on 10/31/022 by Weck Laboratories. Lead was found in 1 classroom sink and 3 drinking fountains. Each classroom has 1 sink and 1 drinking fountain. The facility closed off the sink and drinking fountains with [CONTINUE ON PAGE 2]

plastic wrap and red tape. Lead was not found in bathroom sinks. Director stated children bring their own lunch and meals. Director stated teachers wash dishes in sinks that are free from lead. Director stated the sinks with lead will be replaced on 11/10/2022 and a new lead test will be conducted.

LPA observed 2 children's bathrooms. Bathroom 1 is shared by Kangaroos and Dolphins. Bathroom 1 includes 4 toilets and 4 sinks. Bathroom 2 is located in the Kittens classroom and includes 2 toilets and 2 sinks. There are sufficient toilets and sinks to accommodate the facility’s capacity. Bathrooms were found to be clean.

LPA observed two large outdoor play areas equipped with age-appropriate play equipment that is in safe condition. Outdoor play area 1 is designated for 3 to 5 year olds. Outdoor play area 2 is designated for 2 year olds. The surface of the outdoor activity space is maintained in safe condition and is free of hazards.

The 2A10:BC fire extinguishers are located in each classroom. LPA observed carbon monoxide detectors in each classroom and a fire sprinkler system throughout the facility. Facility was observed to be operating within the conditions, limitations, and capacity specified on the license.



LPA observed staff records to contain appropriate documentation of education credits. All staff are trained in CPR and Pediatric First Aid (expiration date 9/2024) and Mandated Reporter training expires on 2/2023. LPA reviewed a sample of children’s files and observed files were complete.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.
