25 | Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced case management inspection to the above facility. LPA initially met with Hyun Lee, Administrator, who guided analyst on a tour of the facility. LPA was later met met by Su Ahn, Director. There were 11 staff with 55 children upon arrival.
During the inspection LPA observed that there were 7 children with Staff #1 in a room. LPA observed that the children were running around. When LPA asked about the room, the Administrator informed LPA that the room is called the T.V. room. LPA observed that there were no activities, no toys laid out, no television was on and Staff #1 was not engaging the children in care. LPA advised the Director and Administrator to ensure that children have books, toys, and activities to play with and that staff should be engaging the children in care.
In addition, LPA was informed through interviews conducted with staff and children, that there was an altercation that took place between Staff #2 and Staff #3 at the facility in the month of November of 2019 in the presence of the children that were in care. Children interviewed disclosed that they heard doors closing, but did not see staff during the altercation.
Therefore, the following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Su Ahn, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. |
