Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced complaint inspection to the above facility to deliver findings of the complaint. LPA met with Lynette Jones Sawyer, LPA toured the facility. There were 71 children including Toddlers upon arrival.

During the investigation LPAs Powell and Phillips obtained copies of the facility roster, took pictures, interviewed staff and complaintant, and obtained copies of other supportive documentation.
Information provided by the complaintant indicates that Facility released daycare child to an adult which was not on the child's emergency contact list and Staff engaged in a verbal altercation in the presence of daycare children. Staff and Director all made admission to both of the above allegations being true. However the facility released daycare child to an adult who also has children attending facility and on many occasions has signed and picked child in question up from facility.

The emergency contact form was recently changed and the adult name that was on form with permission previously to pick up child was no longer on form. The child was brought back to the facility. Allegation #2 Staff and Director also made admission too. However the Staff was leaving work and was on the facility parking lot and a parent had parked their car incorrect and staff member could not move car out of stall correctly to leave for the day. The staff member and parent did engage in a verbal altercation in front of child. Due to child no longer attending the facility, there is no immediate threat. However, this poses a potential health and safety risk to children in care.

Based on LPAs interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 1 101223 Personal Rights.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Lynette Jones Sawyer Appeal Rights were given.

The facility will also submit a plan on how Emergency Forms will be implemented when updated. Certificates, new implementation and agendas of the trainings will be submitted by POC due date of 3/04/2022.
