On 6/5/2024, Licensing Program Analyst (LPAS) Carol Heath and Crystal Ali met with the Director, Loretta Pilafas, to conduct an unannounced Annual inspection for the infant program. LPAS disclosed the purpose of the inspection and was granted entry to the facility. Upon arrival, LPAS observed 7 Infants (3 months to 1 year old) in classroom 1 and 12 toddlers (1 year old to 23 months) in classroom 2. There were 2 teachers in classroom 1 and 1 teacher in classroom 2 during the nap time. The hours of operation are 6:00 AM - 6:30 PM Monday - Friday. Incidental Medical Services (IMS) were discussed. The center also has a Pre-School and School-age component.
Physical Plant: Furniture and equipment were inspected for age appropriateness and good repair. There are no walkers on the premises. Potty training will be done for this age group upon parent request (toilet training begins at age 2). The bedding is changed daily (washed). The bedding is stored separately and properly inside the cubbies. Floor mats and cots (stored in a locked area) were observed. Used diapers are put in a trash (diaper pail) container with a lid underneath the changing table. Each child will have (already in Infant #1 room) a container labeled (photo and name) with personal items. The infant room is clean and safe. There is a bathroom with one toilet. There is a sink (age-appropriate) near the changing area for children. The sanitation solution is inaccessible to infants (under the sink with lock). Age-appropriate cribs, cots, changing tables, and feeding chairs were clean and in good repair. The staff bathroom is located in the Toddler room. LPAS observed sinks within arm’s reach of changing tables, soap, and paper towels available. Cots are available for napping.

The director states they are sanitized daily (bleach & water), and linens are washed and changed daily. They are stored separately within separate cubbies. *Sign-in and Out sheets were reviewed.
Napping: LPAS observed children's personal sheets in each cubby. Cots are used and maintained in good condition, wiped/cleaned/disinfected (daily), weekly, or when soiled, with no contact with other children's bedding. LPAS observed staff wearing disposable booties in the infant room. Both rooms’ disinfectants, cleaning solutions, poisons, and other dangerous or hazardous items were inaccessible to children and stored by magnetic locks in cabinets.
The licensee maintains the temperature in rooms that children occupy between a minimum of 68 degrees F (20 degrees C) and a maximum of 85 degrees F (30 degrees C).
LPAS observed the first-aid supplies in a location accessible to staff but inaccessible to children. LPAS observed all supplies in each First Aid backpack.
The outdoor: The facility has three playground areas (multiage groups one and two) for the toddler and infant children separated from school-age and preschool children. The outside play area is completely fenced. Outdoor play equipment was inspected for health, safety, good repair, and age appropriateness. There are turf and concrete areas for active play. The area was observed to be free of debris from hazards, holes, broken items, debris, and cushioning material underneath. No bodies of water were observed. There are areas for shade and rest.
Care and Supervision: The center was found to be operating within its specified ratio and capacity. LPAS observed children and contained emergency contact information. The sleeping log was reviewed. The staff followed Safe Sleep Regulation to conduct 10-15-minute checks. All infants have an Individual Infant Sleeping Plan (LIC 9227) in their file.

Staff Records/ Children Records: Children's and staff records were reviewed. LPAS verified that at least one staff member is present with current CPR and First Aid Training (expiration 09/28/2023). All staff complete the Mandated Reporter training every two years. All staff receive the Measles, Pertussis, and Influenza vaccinations. All staff have required fingerprinting associated with the facility.
LPAS observed 5 children's files contained all required licensing documents.
Food Service: The clean, fully equipped kitchen has a refrigerator/freezer, microwave oven, and stove. The facility currently has a food program. LPAS observed a refrigerator in classroom 2 where milk formula is stored for the infants. Parents provide the milk formula for each of their infants.
Menus were observed and posted weekly. Allergy lists with pictures of corresponding children were reviewed and found posted on the door in the Infant room and in the kitchen on cabinets. Food and snacks were reviewed for availability, quantity, proper storage, and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness, and proper equipment (2 refrigerators/freezers w/ temperature logs, sink, microwave, and stove/oven). Food is kept separate from cleaning supplies. There was 1 additional refrigerator/freezer inside of the infant classroom. LPAS observed bottles labeled with the infant's names and current date. Soaps, detergents, cleaning compounds or similar substances shall be stored in areas separate from food supplies.
Other:
· Telephone service, heating, lighting, and ventilation were evaluated.
· The isolation area for sick children is located in the front office.
· A review of medication policy indicated that prescription medication is administered only with the parent's permission. The Director and other administration staff administer medication and document the dosage, date, and time on a log. Medication is brought and taken home by the parent daily. Medication is properly labeled and stored in its original container.

The incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to the publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. The Center does not provide IMS at this time.
Mandated Reporter Training Requirements: §1596.8662 - As of January 1, 2018, childcare providers, administrators, or employees who work in a licensed facility shall complete the mandated reporter training provided according to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.
Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a daycare center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
All Licensing reports are available for review online and are considered public information. Summary: Assembly Bill 2621 added Section 1596.819 to the Health and Safety Code, to require the Department to post certain licensing information for CCCs and FCCHs on its public internet website.

Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state-licensed facility to another using form LIC 9184
Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). In addition; all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of the report documenting a Type A Citation and sign form LIC 9224 acknowledging receipt. Staff is aware of the required forms for children's files and forms that shall be posted after licensure.
Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children.

The Parent Board contains all documents required to be posted according to Title 22 Regulations.


No deficiencies are being cited at this time; the facility complies with Title 22
