Facility is not maintaining a ratio of one teacher supervising no more than 12 children in care (See LIC 809D). LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were not complete with immunization records for pertussis, measles and current documentation of completed Mandated Reporter Training. Menus for breakfast, lunch and snacks are posted at least one week in advance where an authorized representative can view them.
Incidental Medical Services (IMS) are not currently not being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, deficiencies are cited today and technical advisories discussed.
A copy of this report must be provided to the authorized representative of all currently enrolled children and any newly enrolled child for the following 12 months. The acknowledgement of receipt of Licensing Reports (LIC 9224) shall be signed and kept in each children's records.
The notice of site visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will results in a civil penalty of $100.00.
Exit interview conducted, reported and appeals rights were discussed and provided to Director.
