On 06/24/22 at 11:58 a.m., Antonio Almanza, Licensing Program Analyst (LPA), conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Director, Levitt Alana, and toured the facility indoors and outdoors. The facility is operating Monday through Friday from 8:00 a.m., to 3:00 p.m.

Four of Eleven classroom in the facility are currently being used to provide care and supervision to children in care. The facility is using the Blue, Yellow and Green classrooms. Classroom 10 is used as a nap room for children. All classrooms have a bathroom accessible from within. All classrooms are equipped with first aid kit and telephone.
There are no bodies of water on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Solid waste storage containers have tight-fitting covers and are in good repair. Menus are posted at least one week in advance where an authorized representative can view them.

Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Facility has smoke detectors and carbon monoxide detectors in the hallways of the facility. Fire extinguishers (2A10BC or larger) are located in the hallways and were serviced on June 2022.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPA reviewed 10 children’s children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child. LPA reviewed 5 staff files and observed files were complete with documentation of meeting qualification requirements, immunization records for influenza, pertussis and measles. LPA reviewed files and 5 of 5 files do not have the Mandated Reporter training certificate, they are in HRs master files.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Facility is in substantial compliance during todays visit.



A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Levitt Alana.
