1/25/2023 Licensing Program Analysts (LPA) Judy Laureano conducted unannounced Annual Required Inspection at 1511 20th Street, Santa Monica, CA 90404. LPA was greeted by Lori Ayala facility director. LPA toured the facility indoors and outdoors and observed the following:
Allie Alligator Room- 6 children and 2 staff members
Dee Dees Deer Room- 9 children and 2 staff members
Katina Kats Room- 6 children and 1 staff member
Bubba Bears- 8 children and 2 staff members

During today’s inspection, 29 children were present. The preschool serves 54 preschool children ages 2 years old to 6 years old.

LPA toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit:

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. LPA observed barricaded open face heaters in facility.

Disinfectants and cleaning solutions are kept in each classroom, top cabinet making the content inaccessible to the children in care. Additional cleaning supplies and disinfectants are kept next to the kitchen area- locked and inaccessible to the children in care. Director confirmed facility has a designated cleaning crew that comes and clean in the evening and weekend.

Teachers are responsible for the daily cleaning in between classroom activities. Outside pest control vendor is contracted by the facility’s landlord to ensure that facility stays pest free. Facility is free of flies, insects and rodents. No poisons were observed during the inspection.

First Aid Kit and earthquake kit were observed next to the director’s office. Medication is stored in a storage box kept in the kitchen next to the director’s office- making the content inaccessible to the children in care.
Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.

The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment have cushioning material to absorb falls. Teachers ensure playground and outdoor equipment is safe and free of hazard before children use the area.
LPA observed director removed two children’s tricycles that need repair- a handlebar cover. Director agrees to make the item inaccessible until repairs are completed.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility does not provide meals, facility only provides snacks. All food is protected against contamination and any contaminated food is discarded immediately. Families are responsible for ensuring meals are brought to facility in appropriate lunch boxes/bags (e.g. ice packs and/or thermos). Drinking water is available both indoors and outdoors, filter water is provided in each classroom and children are encouraged to bring their refillable water bottle. If children forget their refillable water bottle, facility has water bottles available for children to use. Menus are distributed to families via Brightwheel- online application and a copy is maintain at facility.

Solid waste storage containers have tight-fitting covers and are in good repair. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

LPA reviewed 6 staff files and observed files were complete. LPA advised director to add designated role to each file to assist LPA in reviewing file. Director agrees to ensure all files have updated proof of Flu vaccine and/or waiver, LPA issues a Technical Advisory.

Director’s Mandated Reporter was completed and CPR and 1st Aid completed with an expiration date of 2/2024. Preventative Health and Safety Training was complete by director on 11/20/1999. LPA encouraged director to consider taking the course again as a refresher since LEAD Prevention has been added.

LPA provided facility admin with a copy of LIC311A and LIC 125 to use as a guide to audit files and program.
Facility currently uses an electronic sign in/out system and director is able to print out a copy for LPA’s review. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPAs reviewed 6 children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

LPA discussed Guardian and provided director with resources on how the facility can signed up.

Facility representative, Lori Ayala, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPA and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee facility representative, Lori Ayala.
