Licensing Program Analyst(LPA) Raul Navarro conducted an unannounced random annual inspection on 11/03/2022. LPA arrived at the facility at 09:38 am. LPA met with Licensee Debra Hawkins who guided analysts on tour of the facility. This is a preschool program licensed for 29 preschoolers. Facility operation hours are Monday to Friday from 6:00 AM to 6:00 PM.

All areas identified on the Facility Sketch were inspected. LPA observed 12 children present with two staff. The following was observed during the tour of facility:

Teacher/child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Per Licensee, medication is not administered at the facility.



All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the rest area is used as an isolation area. Mats are available if needed in the office. Parents are contacted immediately when children are determined to be ill.

Lunch menus were reviewed to ensure that they are being posted one month in advance and visible to an authorized representative. The facility provides breakfast, lunch and a PM snack.

All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in all indoor classrooms and drinking fountains are also available outside. Per Licensee, the water has not been tested for Lead.

All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. During today's inspection, LPA did not observe one or more functioning carbon monoxide detectors that meet statutory requirements. The Licensee takes measures to keep the facility free of flies, other insects and rodents.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Licensee states that there are no bodies of water on the premises and the LPA did not observe any bodies of water during this inspection. Areas around and/or under climbing equipment have cushioning material to absorb a fall.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.



Children’s Records were reviewed to ensure that Immunization Records are on file , LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 702- Health History, LIC 701- Physicians Report, LIC 995A Notification of Parents’ Rights, and LIC 613A- Personal Rights.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. During file review. LPA Navarro did not observe the required Mandated Reporter Training on file for all staff present. This is a potential risk to the health and safety of the children in care.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The deficiencies listed on the following page were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Exit interview was conducted with Licensee, Debra Hawkins.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
