Licensing Program Analyst (LPA) Alicia Mooberry conducted a Required Annual Inspection on this date. Upon arrival at 11:10am LPA met with Lee, Bok Rye, Licensee, who provided tour of facility. LPA explained the purpose of inspection and provided the inspection Entrance Checklist, LIC 126. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access, including off limit bedrooms. Per licensee, the hours of operation are Monday-Friday 8:00am-6:00pm. There were 4 children present, including 2 infants. Also present was Kyungjin Lee, Assistant. All adults present have been cleared and associated. Individuals residing in the home were discussed and noted.

This is a single story 3 bedroom 2 bathroom home with detached garage. The front of the home is used as a large classroom and playroom, the enclosed porch is converted to daycare office/parent pick up drop off area.

Off limit areas are: 3 bedrooms, kitchen and laundry room, and detached garage. The kitchen and laundry room are used as passage to the backyard and has a safety gate to make inaccessible at any other time.

Off limit bedrooms and detached garage we observed to be locked and are kept inaccessible during operating hours, per licensee. The licensee does understand that licensing staff may have access to off-limit areas during inspection visit if necessary.

LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form posted in patio visible to parent/guardians of children in care. LPA reviewed completed facility records including; LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan. and Fire/earthquake drill (Last conducted 8/2023)

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged and was serviced on DATE, Licensee was reminded that fire extinguisher needs to be serviced yearly. ------------------Page 1 – Report Continues

LPA provided licensee with an updated Parents Rights Poster PUB 394 with current Regional Office Information.

The home maintains telephone service via cell phone/LAN line. The home is observed to be clean and orderly. There are toys and other age-appropriate material available.


LPA observed Raid and other insecticide cans in overhead wall shelf in laundry room. Licensee removed the raid cans and stored in locked closet by laundry area. LPA observed Laundry room has gate and is inaccessible to children, licensee states they understands that all poisons must be lock, not only inaccessible to children.

LPA discussed with licensee regarding firearms or weapons stored in the home. Isolation area for sick children waiting to be picked up is in the classroom area, supervised and away from the other children.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Currently, children are using the back yard for outdoor play. The outdoor play area was observed to be fenced with age appropriate and safe toys. LPA observed plastic play structure, which is anchored to the ground with cushioned material under structure. The facility does not have a pool or similar bodies of water. Per licensee, there are no pets in the home

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months)

Staff records were reviewed. LPA observed that Staff 1 has Pediatric First Aid and CPR certification that is not EMSA approved. This poses a potential risk to the health and safety of children in care

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA observations and records review one (1) B deficiency will be cited today in accordance with California Title 22 Regulations.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Lee, Bok Rye. Appeal Rights were discussed.
