On 4/13/2023 Licensing Program Analysts (LPAs) Deborah Lowe and Maria Rendon met with Licensee, Ada Kadosh. The purpose of the visit was an unannounced annual inspection at the home to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Homes will be met. The Family Child Care Home operates Monday-Friday 8:00am-5:00pm.

LPAs Lowe and Rendon observed no children in care due to Passover holiday.

Home is observed as a 2-story home with the on limit areas listed: kitchen, daycare room, the bathroom near the daycare room and family room. The off limit areas are the living room, dining room, downstair office, and the second floor, which has three bedrooms and two bathrooms.

Child Care is primarily conducted in the daycare room. Children and parents enter home from the side door in to the daycare room.

LPAs observed two working carbon monoxide and verified smoke detectors in the daycare room and family room. LPAs were able to hear a successful test. Fire extinguisher 3A40BC with a service date 08/03/22 was observed in the daycare room. Electrical outlets were observed to be inaccessible to children in care with the use of plug covers. The home was observed to be clean and orderly with heating and ventilation. LPAs observed safe toys, play equipment, and materials. LPAs verified the home has a working telephone service.

LPAs advised cleaning compounds and detergents shall be inaccessible to children. LPAs advised poisons shall be locked with a key or a combination lock. LPAs observed a locked storage unit for bleach and other cleaning compounds outside with padlock.

Mats were observed for napping. Four play yards were observed to be available for infants

Kitchen was observed with sharp utensils, matches, and open bottles of alcohol are inaccessible. Kitchen was observed to be clean. Per licensee, kitchen is used for additional space for eating when needed only. Licensee provide all foods to the children.

The on limit bathroom near the daycare room was observed to be clean with toilet and faucet safe and operable. Shower is free of hazards.

Outdoor area is located in backyard. Outdoor area was observed to be fenced in.

LPAs advised smoking is prohibited on the premises of a family child care home as specified in Health and Safety Code Section 1596.795(a). A baby walker shall not be allowed on the premises of a family child care home in accordance with Health and Safety Code Sections 1596.846(b) and (c).

LPAs advised, fixtures, furniture, and equipment that have been banned or recalled by the United States Consumer Safety Commission shall not be used for children in care or accessible to children in care.

LPAs observed the following required postings: current facility license, LIC610A Emergency Disaster Plan, and PUB 394 Notification of Parents’ Rights Poster.

LPAs reviewed six childrens records and found records to be complete with emergency cards, immunization records, notification of parent’s rights. LPAs reviewed individual sleeping plans and documentation of 15-minute safe sleep checks.

Review of staff records, LPAs found that staff records were complete with criminal record clearances, immunizations, and employee rights. CPR first aid for licensee and staff was observed with an expiration date of May 2023. LPAs reviewed licensee file and mandated reporter certificate was observed with a completion date of July 2021.

During visit licensee was not able to provide Preventative Health Practices training verification. LPAs advised per CC Regulation 102416(c) Personnel Requirements (c) The licensee and other personnel as specified shall complete training on preventative health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. Facility will receive a type B citation for the deficiency.

During visit licensee was not able to provide Mandated Reporter certificate from staff assistants A1 and A2. LPAs advised per Health and Safety Code 1596.8662(b)(1) Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Facility will receive a type B citation for the deficiency.

LPAs advised per CC Regulation 102417(a) Operation of A Family Child Care Home The licensee shall be present in the home and shall ensure that children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

A notice of site visit was given and must remain posted 30 days.
