32 | Disclosures were made that rat droppings have been observed by staff in different areas of the facility, such as the carpet in the Toddler Playroom and area between the Toddler and Tiny Tots room on several occasions during the last month. Disclosures were also made that the facility did catch a rat with a mouse trap at the facility recently. However, during today's inspection LPA did observed a clean facility their were rat droppings around the trash can where dirty diapers are disposed from the Toddler Class room (photos taken). LPA also observed a large mouse trap in the same area of the facility (photos taken). Per disclosures made by staff during inspection of facility and during interviews conducted, and observations made by LPA, it was determined that the facility did have a rodent in the facility and rat droppings have been seen in facility where children play sleep and eat.
Based on the LPAs observations, interviews concluded the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, Title 22 101238(a)(1) Buildings and Grounds, is being cited on the attached LIC 9099-D.
A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). Acknowledgement of Receipt (LIC 9224 form) must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports
Exit interview was conducted with Director, Elenita Mercer. Notice of Site Visit was given during inspection. A copy of the report LIC809 and LIC809D with Appeal Rights were discussed and will be given to facility. |
