On 05/01/2019 at 10:40 am, Licensing Program Analyst (LPA) Sabrina Martinez arrived at Smart Start located at 2505 Lincoln Blvd., Santa Monica, CA 90405, for the purpose of delivering the investigation findings of the above mentioned allegation.

Based on the information obtained throughout the course of the investigation, it was revealed that the facility was utilizing a Little Tikes Giant Slide Bouncer. Per the equipment's manufacturer's specifications and instructions, it is designed for children ages 3-8 years old and the giant slide bouncer is intended for outdoor domestic family use only and not for use in public areas or as a rental. Therefore, the allegation that facility has unsafe play structures for children in care is substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. The facility is issued a Type B citation today, 05/01/2019. (See LIC 809-D for deficiency cited). An exit interview was conducted and a copy of this report, appeal rights and Notice of Site Visit were provided to Sharon Lowery, licensee.
