On 11/28/2022, 10:35AM, Licensing Program Analyst (LPA) Denise Miranda, conducted an unannounced Annual Required Inspection for the preschool License. LPA met with Jessica Sperber, Director and informed the purpose of the visit. LPA was guided by Director and toured the facility indoors and outdoors. Days and hours of operation are Monday-Friday 7:45 AM – 3:00 PM. LPA, observed 10 children in care and 2 staff and Director. Facility checklist (LIC 125) provided upon arrival to Director.

There were two classrooms (classroom#Preschool and Pre-Kindergardent). During this inspection, LPA observed the classroom Pre-Kindergarten is using for storage. Facility has a designated to the preschool program on file. Facility is located on the Our Lady of Peace Elementary School Campus.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space needs to be replace. LPA observed at the playground by the tree area, needs to be cover with sand, wood cheap and make even. LPA advised Director to place a color tape by the cushing area indicate that has a level.

The restroom is located inside of the classroom. LPA inspected the bathroom and observed four toilet and 3 sinks. LPA was noted to be clean and equipped with good flushing toilets, soap dispenser, paper towels.


Outdoor activity area is fenced all around and is supplied with age and size appropriate

equipment. An adequate amount of cushioning material turf is in place under play
equipment. Adequate shade is provided in outside play area. Drinking water is provided in the outdoor play area via gallon. Per Director the water fountain located at the playground is out of service.

Per Director, facility provided the morning and afternoon snacks and parents send their own lunch or hot lunch will be provide by third party. LPA observed that facility has available food. LPA observed, small refrigerator and microwave inside of the classroom. This area are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors.

The facility is free of flies, insects and rodents. Facility has one functioning carbon monoxide detectors that meet statutory requirements and fire extinguisher.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the preschool child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 preschooler per full qualify teacher. LPA reviewed a sample of 5 children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed all staff files present during this inspection and observed files were not complete. Menus are posted at least one week in advance where an authorized representative can view them, however, facility missing to provide the amount and portions on their menus.

transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. Per director no currently children needs medical services.


Regulation and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to: California Department of Social Services | Community Care Licensing Division | 300 N. Continental Blvd. Suite 290-A |El Segundo, CA 90245

Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC’s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test.

LPA provided the Director with the Provider Information Notice (PIN) 21-21CCP and Lead Testing in Child Care Centers Frequently Asked Questions.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
