25 | ***THIS IS AN AMENDED REPORT TO CORRECT CENSUS OF CHILDREN IN FACILITY. THERE WAS A TYPOGRAPHICAL ERROR IN TOTAL CENSUS IN ORIGINAL REPORT*** THE FACILITY WAS MADE AWARE DURING OFFICE MEETING WITH THE DIRECTOR ON 10/29/19*** On 6/3/19 at 9:00 AM, an unannounced case management inspection was conducted by Licensing Program Analyst (LPA) Helen Estrella, in conjunction with a complaint inspection. Upon arrival, LPA met with Teacher Ratnasekareand LPA informed her the nature of the visit. The Center Director Shirani Perera arrived to the facility 35 minutes later. There was a census of 43 preschool children and 7 toddlers with 3 teachers, 1 aide upon arrival.
The following deficiencies were observed during today's visit:
On 6/3/19, at 10:36 AM, LPA conducted another census and observed the facility census with a total count of 7 toddlers and 43 preschool children with 3 teachers, 3 teacher assistants and the Director. This brings the total census of the facility to 50.
At 9:30 AM, LPA observed Adult #1 arrive to facility and working in the facility, assisting in Classroom C&D during during today's inspection. Adult #1 states she does not remember when she started working at the facility, but knows it has been less than a month. Adult #1 states has not completed Live Scan process and it was confirmed she does not have a Criminal Record Clearance and/or was associated to the facility. LPA requested Adult #1 to be removed from the facility and the Director was informed that all adults/employees/volunteers must obtain a Criminal Record Clearance/exemption prior to working/assisting with children and being in the facility. LPA reviewed Adult #1 file.
From 10:36am to 11:30am, LPA observed Classroom C&D with 17 children and to be observed by 2 aides and no qualified teacher at this time. The Director states Teacher called out for the day and another qualified teacher arrived to the facility at 11:30AM.
At 11:27AM, LPA observed 3 preschool children in the restroom alone, unsupervised playing with water. LPA informed aide whom assisted children to the classroom.
Between 12:07 PM to 12:55 PM, LPA observed 5 preschool children picked up from the center and the facility's capacity totaled 45 children bringing the facility to compliance. |
