32 | LPA Hernandez continued the tour of the facility with the Assistant. LPA Hernandez observed 2 children restrained in highchairs with no food and no adult present. LPA Hernandez observed all off-limit areas identified on the facility sketch accessible to children. (photo taken) Per Assistant, off-limit areas were accessible because a deep cleaning of the home was being conducted. LPA Hernandez observed an off-limit bedroom with the door open and accessible. LPA Hernandez observed bottles of alcohol displayed and accessible on a side dresser in the bedroom (photos taken). During the tour of the back yard, LPA Hernandez observed the kitchen door opened and bottles of cleaning solutions accessible photos taken). LPA Hernandez also observed 3 buckets of stagnant water with insects in and above the water (photos taken). LPA Hernandez observed a charcoal grill, a barbeque grill accessible in the backyard where children were playing. LPA Hernandez also observed 2 propane tanks, one of which was connected to the barbeque grill (photos taken). The facility was not operating as specified in section 102417(g). This poses an immediate health, safety or personal rights risk to persons in care.
LPA Hernandez observed alterations to the home. Per Assistant, the facility is being repainted, and the electrical system is being updated. Per Assistant, the detached activity room is also having the electrical system updated. LPA Calvillo did not find records of Licnesee reporting alteration to the home to the Department noted prior to alteration being done. The facility was not operating as specified in section 102416.3(a). This poses a potential health, safety or personal rights risk to persons in care.
Based on observations by the LPAs, Assistant’s self-disclosures, and Assistant’s written statement, the deficiencies listed on the attached LIC 809D page are being cited.
LPAs informed the Assistant that Type A citation(s) shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care.
LPA Calvillo informed facility representative Catherine “Collette” Arriaga that this report dated 08/22/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care
Also, LPA Calvillo informed the facility representative to provide a copy of this licensing report dated 08/22/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for
