This agency has investigated the aforementioned allegation and there is not a preponderance of evidence to support nor deny that staff failed to keep the facility free from pest. Evidence shows that the facility has a pest and property maintenance contract that provides monthly and as needed maintenance for classrooms, entrance area, lobby, restrooms for the facility five (5) days per week. Staff revealed daily maintenance is conducted to ensure best hygienic practices of the facility throughout the day. Therefore, the allegation is deemed Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
A copy of this report, appeal rights, and a Notice of Site Visit were provided. Exit interview conducted.
