On 06/29/2023 Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced annual inspection to the above facility. LPA arrived at the facility identified self and met with Anna Mayes, Licensee, who guided analyst on a tour of the facility. LPA provided the Licensee with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a School Age Program. The facility also has a Preschool license #197409872 at the same location. The Center is a home that is commercially zoned as a Center. The Center utilizes 3 rooms for the school age program. The 2 rooms adjacent to the kitchen are used for activities for the school age program and separated from the preschool program with a gate. The room to the rear of the Center is utilized as the homework area. The school age children use the restroom located in the rear of the Center adjacent to the back room. Hours of operation are Monday through Friday providing care 24 hours a day.

LPA observed 1 child and licensee present when LPA arrived. All individual’s present have obtained a criminal record clearance.

The following was observed during the tour of the facility:
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Age-appropriate sinks and toilets were inspected for availability and good repair in the restroom. Availability of indoor drinking water (bottle water) was observed in the facility.

Disinfectants, cleaning solutions, medication (Licensee states no medication in facility at this time) and other items that are dangerous to children, were inaccessible underneath kitchen sink cabinet which has a key lock. Disaster drill log was available, last drill was conducted on June 2023. Smoke and Carbon monoxide detectors were observed and tested during inspection both were operable. The Center is equipped with a fully charged fire extinguisher which is at least a 2A:10BC. LPA did observe First Aid supplies in the classroom.

LPA observed License and Pub 393 Notification of Parents Rights posted on a bulletin board in kitchen area. All other required documents were not posted during inspection.

LPA did not observed menus posted. Menus for the past 30 days were not available upon request. The facility provides snacks to children only due to the time children are in care at this time. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.



Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. LPA observed the climbing equipment to be dirty/dusty during inspection. The porch of the facility provides a shaded area for the children in care. Availability of outdoor drinking water was observed by bottle water. LPA advised that no children shall be left without the supervision of a teacher at any time. Per licensee the outdoor space is shared with the Preschool children. The children alternate the yard. Waiver on file and posted.

LPA observed toys and playthings are safe, orderly and age-appropriate for the children. Outdoor play area is fenced in and also is free from defects or dangerous conditions.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free. Per licensee, there are no firearms stored on the premises. LPA did not observe pools or bodies of water at the facility.

Children’s Records were reviewed. Children's roster was not up to date during inspection. Sign-In and Sign-Out sheets were not completed during inspection.

Staff Records were reviewed. There were one staff file in the facility for LPA to review. Licensee and staff did not have proof of the AB 1207 Mandated Reporter Training certificate on file. www.mandatedreporterca.com

Staff present did have proof against TB, measles, pertussis, and influenza.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Per licensee all ill children will lay on a mat in the isolation area which is located in kitchen nook area next to cubbies to wait for child to be picked up by their representative.



Criminal Record Clearance - Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA will refer the facility and licensee to the Technical Support Program (TSP) to help providers achieve and maintain compliance with laws, and regulations.

Director was informed that Fire disaster drills are to be conducted at least once every 6 months and a log must be kept. Director also informed children records and facility roster must be kept for 3 years and advised all public reports must be kept for review.

Lead Testing - Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was not completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
