On 12/29/2022 Licensing Program Analysts (LPA) Judy Laureano conducted unannounced Annual Required Inspection at 1536 S. Robertson Boulevard, Los Angeles, CA 90035. LPA was greeted by Minde Ornelas, facility director. Facility owner, Feye Yemini, was also present during today’s inspection. LPA toured the facility indoors and outdoors and the following was observed:

Classroom 1-Toddler classroom- 0 children present

Classroom 2- Preschool Classroom- 10 children and two staff members.
Classroom 3- Preschool Classroom – 14 children and three staff members.

Facility currently operates Monday through Thursday from 8:45 a.m to 3:45 p.m. and Friday from 8:45 a.m .to 1:00 p.m. The preschool serves 28 preschool children ages 3 years through 5 years old, and 12 toddler age children ages 18 months through 30 months. Facility has a total capacity of 40 children.

Toddler component are children ages 18 months to 23 months, and currently facility does not have any children in the toddler component.

LPA discussed with director and owner regarding updating license comments to reflect the ages the facility is currently operating. LPA informed both owner and director that the use of classroom 1, if keeping the toddler component, is only to be used for the toddlers in the toddler component only and cannot be used for the preschool children.

LPAs toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit:

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.

Disinfectants and cleaning solutions are kept in each classroom, top cabinet making the content inaccessible to the children in care. LPA observed cleaning compounds in the kitchen that is use by staff only. First Aid Kit was observed in the main office and in each classroom. Medication is stored in the top hook of the classroom. Currently facility does not have any children that require any type of medication (epi pen and/or inhaler).

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.

The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment have cushioning material to absorb falls. Teachers ensure playground and outdoor equipment is safe and free of hazard before children use the area. Sandbox area is covered at the end of each day.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility does not provide meals and/or snacks, families are required to provide for their children enrolled. Families are responsible to bring in snacks and meals in proper containers such as thermos and or lunch bags with ice packs. Facility does have snacks available in case of an emergency. All food is protected against contamination and any contaminated food is discarded immediately. Drinking water is available both indoors and outdoors, filter water is provided in each classroom and children are encourage to bring their refillable water bottle.

Facility has staff that is responsible for daily cleaning and sanitizing an outside pest control vendor is contracted if facility has a pest problem. Facility is free of flies, insects and rodents. Teachers are responsible for the daily cleaning in between classroom activities. No poisons were observed during the inspection.

Solid waste storage containers have tight-fitting covers and are in good repair. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

LPA reviewed all staff files and observed Mandated Reporter Training was completed by Director- 12/11/2022. LPA issued an advisory and director agrees to have all staff files completed with all necessary document. LPA provided director with a copy of LIC311A and LIC 125 to use as a guide to audit files and program.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.


LPAs reviewed children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

Facility representative, Minde Ornelas, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently NOT being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPAs and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee facility representative, Minde Ornelas
