On 11/17/2022, Licensing Program Analyst (LPA) Carol Heath met with the Facility Director, Jessica Kieffer for the purpose of conducting an unannounced Annual/Random inspection. LPA toured and inspected the area by the facility sketch. The preschool consists of 3 classrooms. The facility also has an infant center. The daily hours of operation are from 6:00 am – 6:30 pm. Teacher/Child ratios are met according to Title 5 of the California Code of Regulations and Title 22 Regulations. Upon arrival, LPA observed 3 classrooms in use with a total of 29 preschool children and 8 teachers on the premises. Incidental Medical Services (IMS) were discussed. Pre Facility Director, there is no children need IMS at this time.
· Physical Plant
LPA toured and inspected the preschool classrooms. Furniture and equipment were inspected for age appropriateness and good repair. Potty training is conducted in the Two Year Old's classroom. LPA observed a sufficient quantity of cots available for napping children. Bedding and linens were individually stored so that each child's bedding is identifiable, and no child's used bedding comes into contact with other bedding. The sheets and blankets are washed weekly (sent home with parents to wash or staff wash after hours) and cots are sanitized daily.

LPA observed cleaning solation (bleach water and Windex) accessible to the children. Trash cans do not have lids. TVs wires are accessible to the children. Unused plugs are accessible. The tall bookshelf are not secure to the wall. The classroom refractor has children and staff’s snack or meals without names. LPA also observed a floor fan on the top of the shelf with dust and uncover.

The telephone service was verified. Heating, lighting, and ventilation are adequate. There are cubbies for children's belongings in each classroom. LPA observed age-appropriate toys and materials. There are fire alarms and carbon monoxide detectors located by the doors of each classroom. Fire extinguishers are maintained properly.

LPA inspected and observed 2 clean bathrooms (boys, and girls). The boy's bathroom has 4 toilets, 1 urinal, and 5 sinks. The girls have 5 toilets, and 5 sinks, however, 1 toilet has no water tank and will need to repair. Sinks are functioning properly and are age appropriate. There is an additional bathroom (one toilet, one sink, changing table) in the hallway next to the boy's bathroom that is available also. LPA observed soap, toilet paper, and paper towels readily available.

· The outdoor play equipment was inspected for health, safety, cushioning material, good repair, and age appropriateness. The play area has sand and concrete. There is an area for rest (canopy). LPA asked how the children get their drinking water. Pre director, children will get the water before or after outdoor play. Some children bring their own water bottle. The play area was inspected and found several outdoor play equipment have peeling pain and the storage shed has some water damage and black mark on the wood. Separate playground schedule for school-age and preschool children. The jungle gym is anchored. There is a bike path, jungle gym, and other age-appropriate toys.

LPA observed a first aid kit. The tweezer is missing in the first aid kit, fully charged fire extinguishers; carbon monoxide detectors throughout the center. There is a Fire Drill/Earthquake log posted in the office and a current Roster available.

· The parent board was reviewed and has all the required forms posted. Fire/earthquake drills current. Children are inspected for illnesses as they arrive. A review of medication policy indicated that prescription medication is administered only with the parent's written permission. The Director administers medication and documents the dosage, date, and time on a log. Medication is brought and taken home by the parent daily. Medication is properly labeled and stored in its original container. There are currently no children in care with IMS needs. There is a separate area for the isolation and care of ill children in the director's office. There is a mat available for ill children. An isolated toilet and sink are available for ill children in the staff restroom.

Sign-in and out sheets were reviewed. The facility uses paper sign-in sheet. The Teacher to child ratio was observed and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. LPA observed a parent board consisting of licensing-required postings. Fire/earthquake drills current.

· Staff Records/ Children Records
Children's records and staff records were reviewed. LPA reviewed 3 staff's files and verified there is at least 1 staff person present with current CPR and First Aid Training (exp. 5/14/2023). 2 staff complete the Mandated Reporter training every 2 years. 2 staff receives Measles, Pertussis vaccination. The director will need to have all staff submit Flu shot record or waiver. There is a staff's member's fingerprinting does not associated with the facility.
LPA observed 10 children's files contained all required licensing documents.

· Staffing Ration and Capacity
Upon arrival, LPA observed 2 classrooms in use with a total of 31 preschool children and 5 teachers on the premises.
· Food Service
There is a clean, fully equipped kitchen (off-limits) with a refrigerator, freezer (1), and microwave oven. Allergy lists are posted in the classroom. The facility provides breakfast and two snacks, children bring their lunch. Menus are posted in the office in each classroom. LPA observed an appropriate amount of food and snacks. LPA observed an appropriate amount of food and snacks. Cleaning solutions and other chemicals are kept in a cabinet above the sink with the lock-in classroom and the main building's maintenance room. Soaps, detergents, cleaning compounds, or similar substances are stored separately from food supplies.

Drinking water is available inside the classrooms in the form of water pitchers and disposable cups. LPA observed a sufficient quantity of cots available for napping children. The bedding is stored separately and properly inside the cubbies.

Menus are posted at least one week in advance and made available for review by the parents.

The following general information was discussed during this inspection:

The incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to the publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. The Center does not provide IMS at this time.

Mandated Reporter Training Requirements: §1596.8662 - As of January 1, 2018, childcare providers, administrators, or employees who work in a licensed facility shall complete the mandated reporter training provided according to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Staff was informed that they need to take mandated reporter training. Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a daycare center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state-licensed facility to another using form LIC 9184

**Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). In addition; all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of the report documenting a Type A Citation and sign form LIC 9224 acknowledging receipt. **Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements


Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children.

Deficiencies cited: (See LIC 809D). The following Type A and B deficiencies are being cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety codes.
