On 12/7/2022, Licensing Program Analysts (LPAs) Carol Heath and Joselito DelMundo met with the facility Director Jessica Keiffer and conducted a Required One Year inspection. LPASS toured and inspected the Infant area in accordance with the facility sketch. LPASS observed 6 infants upon arrival. There were 2 teachers on the premises and the director. The facility consists of one infant classroom. The Center was also licensed for a preschool center in the same building (197410438). The facility operates from 6:00 am to 6:30 pm, Monday through Friday. The incidental Medical Services (IMS) policy was discussed.
Indoor/Infant Area: The center is clean, safe, sanitary, and in good repair, there are no potential hazardous areas observed (passageways), floors are clean and safe, cleaning compounds are inaccessible, poisons are locked, furniture and equipment in good condition, free of sharp, loose or pointed parts, including cribs, cots, changing table and feeding chairs, no insects or flies observed, play equipment are age appropriate. There are bins labeled with children's names for their belongings. High chairs and feeding tables are in good repair. A Sanitary solution was observed on an upper shelf by the sink. The diaper-changing table was observed within arm's reach of the sink. A diaper trash container with a lid was observed outside. LPAS observed age-appropriate toys and materials. Bottles/cups are labeled. Drinking water is available for infants. There is a sufficient amount of cribs available. The sleeping area is separated from the play area by a divider. The Center has an evacuation crib. The bedding is stored separately. Per the director, sheets are cleaned each Friday.
The infant room has a refrigerator with items inside properly labeled. Medication is kept in a locked container. There is a separate adjacent room for additional space for play. The infant room has a Daily Activity Record (meals, medication, diaper changing, napping, activity, arrival and pick-up time) for each child. LPASs also observed the Sleeping Log for each infant. Potty training is conducted in the Two Year Old's classroom. First Aid supplies are stored in the classroom. There is a fully functional smoke alarm system in the facility. A private company cleans the facility daily.

Physical Plant: There are no bodies of water on the premises. No firearms on the premises. Disinfectants, cleaning solutions, poisons, and other items that are dangerous to the infant are inaccessible (top cabinet). LPAs observed fire extinguisher (2A10BC) and smoke detectors are in operable condition. The facility has one or more carbon monoxide detectors that meet statutory requirements.
Napping: LPAs observed cots in a safe condition, cribs are in regulation (meets the requirements of 101439.1(a)-(f), no pillows, crib bumpers, loose bedding), sanitize after each use, cots spaced to have access to a safe walkway, bedding is not in contact with other bedding.
Restrooms: Not for infants.
Outdoor: Outdoor play area is only for preschool age children.
Health-Related Services: Medications when used are inaccessible to children (locked in kitchen refrigerator), child name and date on medication, parent has written consent as required.
Food Service: Food is prepared and served safely, food preparation is completed in the kitchen, cleaning compounds are kept in the supply room (off limits), utensils are cleaned and sanitized after each use, refrigerator temperature is regulated, menu posted in the kitchen and classroom, breakfast and snack provided by the parent. Bottles, dishes, and containers of food brought by the infant's authorized representative are labeled with the infant's name and current date.
The menu is posted. Allergy lists are also posted. Parents have the option to provide food and water for their children.
Staff Records: LPAs reviewed files for Director Qualifications, immunization, and licensing forms (LIC9108, Mandated Reporter, LIC508, LIC308).
Facility Records: LPAs observed the Roster to be complete and current, staff has current CPR/First Aid expires 07/24/2023, and fire and earthquake drills are conducted monthly. The facility has an Infant individual feeding plan, Infant Needs and Services Plan, and Sleep Plan for each infant.
Supervision: LPAs observed infant/child is not left without the supervision (visual) of a teacher at any time, LPAs observed the facility complies with the staff-infant ratio (one teacher-four infants in attendance).
Posting Requirements: Center has required posting (license, PUB 393L, car seat law, emergency disaster plan, earthquake preparedness, lead poison, safe sleep).

Information Discussed:
· IMS (Incidental Medical Services). No infant with IMS need at this time.
· Licensee [or facility representative] was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to the initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

· LPAs discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAS also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

· A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

· LPAs (name of analyst) informed licensee [or facility representative] (include name) that this report dated (insert visit date) document(s) (number of Type A citation) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPAS (name of analyst) informed the licensee [or facility representative] to provide a copy of this licensing report dated (insert visit date) that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
